two violations of 10 CFR 50, Appendix B Criterion Y requirements.

In your letter PGE acknowledged the first violation regatding the use of unapproved guidelines for acceptance criteria when performing 'cisual (VT-3) inspections.

The justification provided, however, was inadequate in that it did not provide sufficient information to support your position that VT-3 reports were evaluated in accordance with NDP 600-1, Control of Nonconforming Parts and i

Components.

For example, your response did not mention that during the initial stages of the inspection period that specift: NCR's were generated for each

  • 'T-3 report which identified a flagged item. Nor did your response exp1Lin that these NCRs became so numerous that you decided it would be more prudent to issue one generic NCR to track and disposition these VT-3 reports. This additional infonnation was reviewed by Mr. W. Wagner of my staff during an inspection conducted at the Trojan plant on October 24-77, 1988. Based on e
