dated April 7, 1986 In accordance with our referenced letter, this submittal provides the follow-up report of fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in April 1986.

Fire doors found inoperable during the April routine surveillances are identified in the Attachment and include: 7 latching mechanism failures; 9 closure mechanism failures; and 1 door blocked open typically for construction, operation or maintenance activities. As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement 'a', were established. Door mechanism failures are being corrected on an "as found" basis.

The responsible party was not identified for the 1 fire door found blocked open.

Mr. J. B. Martin This monthly follow-up report and those sent previously provide updated information associated with an ongoing condition initially reported in 1983.

The follow-up report covering inoperable fire doors identified in May 1986 will be submitted by June 16, 1986.
