Nuclear Regulatory Commission and Mr. Dickinson Smith of the Peach Bottom Atomic Power Station on October 18, 1988, we request that you investigate the allegations regarding a UE&C-Catalytic contractor who indicated that his quality exit interview notes were lost following his termination, and about concerns he raised in that interview regarding orders not to follow Radiation Work Permit c
procedures by his supervisor.
