s ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-327, 50-328 Sequoyah License Nos. DPR-77, OPR-79 During the Nuclear Regulatory Commission (NRC) inspection conducted August 6 through September 12, 1988, violations of NRC requirements were identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions " 10 CFR Part 2, Appendix C (1988), the violations are listed belw:

A.

Technical Specification 6.8.1 requires that written orocedures shall be established, implemented and maintained covering the activities listed in Appendix A of Regulatory Guide 1.33 Revision 2, including surveillance and test activities of safety related equipment.

Contrary to the above, licensee personnel failed to follow SI-82.2 in the proper sequence while performing work on radiation monitor 2-RM-90-106 on August 31, 1988.

As a result, Unit 2 experienced a Containment Ventilation Isolation when a test signal was injected into the monitor.

Contrary to the above, licensee personnel f ailed to follow SI-83 on Septerrber 3, 1988, while performing work on 0-RB-90-101. As a result, an Auxilitry Building Isolation occurred.

l This is a Severity Level IV Violation (Supplement 1).

B.

Technical Specification 3.3.1 requires the reactor instrumentation listed j

in table 3.3-1 to be operable and that table notation 6 be entered as the Action Statement when less than 4 channels of Overpower Golta T Trip or less than 4 channels of Overtemperature Delta T Trip are.,cRABLE.

Technical Specification 3.3.2 requires the Engineered Safety features Actuation System (ESFAS) instrumentation channels shown in Table 3.3-3 to be OPERABLE aad that table notation 16 be entered as the Action Statement when less than 4 channels are OPERABLE.

Table notation 16 requires the inoperable channel to be placed in the tripped condition within I hour in order for power operatior, to continue.

Contrary to the above, reactor trip channels for Overpower Delta T i

This is a Severity Level IV Violation (Supplement I).

C.

Technical Specification 6.8.1 requires that procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, be established, implemented, and maintained.

This includes maintenance procedures.

Contrary to the above, Work Plan 7152-01 was inadequate and not properly implemented in that perfornance of the Work Plan on August 4, 1988 resulted in a load shed of IB-B 6.9 kv shutdown board and a loss of the board for approximately 20 mit.utes.

This is a Severity Lovel IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Director, Of fice of Special Projects, and a copy to the NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly markeo as a "Reply to a Notice of Violation" and should include (for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the I

corrective steps which have been tsken and the results achieved, (4) the I

corrective steps which will be taku to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
