Please provide the following information to complete your amendment application for NR-345-D-104-S.
1.
LABEL:
The device's external label includes the sealed source's model and serial numbers.
Upon replacement of a sealed source, where would a specifically licensed person acquire a replacement label containing all correct information? Since the aluminum label is riveted onto the device, how would the label be removed and reattached?
2.
EXTERNAL RADIATION LEVELS:
The table of external radiation levels appears to have a conversion error in converting mr/hr to Sv/hr. The numerical value of the Sv/hr should be ten times greater than the numarical value of mr/hr, not the other way around. Please clarify this conversion.
3.
QUALITY ASSURANCE AND CONTROL:
What is the minimum interval for the on-site inspection of the manufacturer to confirm that the device is manufactured according to the statements and representations contained in their application?
