EOM&LO, Juneau Laboratory ATTN: Alan Love 10107 Bentwood Place Juneau, Alaska 99801-8552 j SUBJECT: NOTIFICATION OF CESSATION OF LICENSED ACTIVITIES By telephone conversation on October 7,1998, you informed the NRC of your intent to cease licensed activities and terminate your Radioactive Materials License No. 50-14858-02. The NRC has specific requirements as specified in 10 CFR 30.36 for decommissioning facilities and for terminating licenses. The information outlined below may be helpful in understanding these requiv ments. Licensees that used only sealed sources, and the most recent leak test demonstrates that the sources did not leak while in the licensee's possession (i.e., if leak test results have been <0.005 pCi); or used relatively short-lived radioactive material (i.e., T% s 60 days) in an unsealed form and the maximum activity authorized under the license has decayed to less than the quantity specified in 10 CFR Part 20, Appendix C should: 1. Demonstrate to NRC that the sources never Inked, both while in your custody and upon arrival at the ultimate destination (i.e., L; leak test records) or that the maximum activity of radioactive material authorized under your license has decayed to less than i the quantity specified in 10 CFR Part 20, Appendix C; 2. Dispose of or transfer the licensed material in accordance with NRC requirements; 1 3. Transfer the records discussed in 10 CFR Parts 30.35,30.36, and 30.51 to the NRC Region IV office; and 4. Submit an NRC Form 3#" Certificate of Disposition of Materials" to NRC. If you possessed and used only sealed sources, but cannot demonstrate that the sources did not leak while in your possession (i.e., leak tests are not available or indicate contamination >0.005 pCi); or possessed unsealed radioactive material with T% s 60 days but the maximum activity authorized under the license had not decayed to less then the quantity specified in 10 CFR Part 20, Appendix C; or possessed unsealed radioactive material with T% > 60 days but s 120 days you should: 1. Determine the extent of contamination at your facility; 2. Remove residual radioactive material to levels that would permit release of the facility;

Dispose of the licensed materialin accordance with NRC requirements, usually by returning sealed sources to the manufacturer or disposing of licensed material as outlined in the NRC regulations; 4. Determine the radiological status of your facility and perform further remediation, if ) necessary to meet NRC's criteria (See Enclosure 3 for a description of the surveys that should be performed for licensees rneeting the criteria discussed above); 5. Submit the results of a survey of your facility demonstrating that residual radioactive materiallevels meet the NRC's criteria for unrestricted use or demonstrate that your facility, or portion of the facility, meets NRC's criteria for unrestricted use to NRC by I some other means; j 6. Transfer the records discussed in 10 CFR Parts 30.35,30.36, and 30.51 to the NRC, Region IV office; and 1 7. Submit an NRC Form 314 " Certificate of Disposition of Materials" to NRC. ] ! 8. Hemandez, This is to inform you that the process of disposing of radiation sources is completed and we now request termination of Radioactive Materials License No. 50-14058-02. Attached find supporting documents for the disposal of these materials and for the post disposal survey.

Pads used to absorb a spilled solution of Americium 241, (A sp"I which occurred in 1991), had been stored in the
i same location as the standard sources. These pads were rinsed with 1% Nitric Acid three times, and with water I three titaes. The rinsates were collected, combined, neutralized and diluted to 20 liters. This solution was then i '
disposed of into a municipal sewage treatment plant. The total activity is estimated at 10 pCl, with a maximum of 15 pCl. The remaining sources were sent to ATG in Richland, WA for disposal. See Form 314 for details. l
- The survey was carried out by wipe tests. The ' wipe disks were then sent to Ouanterra in Richland for counting.

The results were returned in DPM. The results are listed in the tatle below. The sources were stored inside a locked metal box, which was stored in a vented metal cabinet located inside a locked storage room. The survey was taken and then the sources were packed for shipment. Actual shipmen', was delayed while the State of Washington processed permit applications. All sources were sealed. Each source was placed in a container which was then packed into the metal box with absorbent material. The metal box was appropriately labeled and packed into a larger box which met DOT standards for the shipping of exempt quantities. All documentation is attached.
