QUAD CITIES INSPECTION REPORT 50-254/99001(DRP);
50-265/99001(DRP)
Dear Mr. Kingsley:
This refers to the inspection conducted January 21 through March 6,1999, at the Quad Cities
i
Nuclear Power Station. The purpose of the inspection was to determine whether activities
authorized by the license were conducted safely and in accordance with NRC requirements.
The NRC conducted routine resident inspections in this period. Based on the inspectors'
findings, overall performance at the Quad Cities Station was safe. At the conclusion of the
inspection, the findings were discussed with members of your staff.
Based on the findings of this inspection, the NRC has determined that two violations of NRC
requirements occurred. The circumstances surrounding the first violation are described in
,
detail in the subject inspection report. The violation was of concern because it involved a failure
to follow station procedures for the use of shutdown cooling. The result of the failure was the
loss of approximately 7000 gallons of inventory from the Unit 2 reactor vessel while the unit was
shut down. Several barriers which could have prevented or minimized the event were not
effective. Standards for procedure adherence, communication, control room briefings, control
room turnovers, self checking, and peer. checking were not maintained during a very aggressive
outage work schedule. In addition, scheduling long working hours for operators was a routine
practice during this period, and the licensed operators involved with this event were well into the
second part of a 16-hour shift when the errors occurred.
Your response to this event was prompt, aggressive, and thorough. A corporate-led team
involving representatives from each of the stations was formed to investigate the error and the
contributing circumstances. The Quad Cities Station used the investigative team's findings as

well as local's'7 tion observations to develop the resultant corrective actions. Accordingly, this
'
issue is being treated as a Non-Cited Violation (NCV), consistent with Appendix C of the
Enforcement Policy. If you contest the violation or severity level of the NCV, you should provide
a response within 30 days of the date of this inspection report, with the basis for your denial, to

The second violation involved a failure to follow station out-of-service tagging procedures
concerning a shutdown cooling isolation valve. The failure resulted in damaging a safety-
related motor-operated valve that was required to permit decay heat removal for Unit 2.
Previous examples of out-of-service tagging related problems were identified in inspection
Report 50-254/98023; 50-265/98023, including Notice of Violation 50-254/98023-01. This
second violation was considered an additional example of the aforementioned Notice of
- Violation, for which corrective actions had not yet been fully implemented. Therefore, no Notice
of Violation is being issued for this second violation. Your response to Notice of
Violation 50-254/98023-01 should be modified, if necessary, to include corrective actions
sufficient to address the out-of-service tagging errors identified in this report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, your
response if you choose to provide one, and its enclosure will be placed in the NRC Public
Document Room.
