Special Inspection 50-352/85-43
This refers to the special inspection by Mr. E. M. Kelly on November 1 - 6,
and November 13 and 14, 1985 at the Limerick Generating Station, Limerick,
Pennsylvania. The inspection consisted of a review of the circumstances sur-
rounding the discovery of both Emergency Service Water (ESW) system Loop B pump
discharge valves locked in a throttled position at 8 p.m. on October 31,1985.
This inspection also addresses the discovery of an ESW Loop A pump discharge
valve in a fully open but unlocked condition on November 14, 1985. The findings
of this inspection were discussed with Mr. G. M. Leitch and other members of
, -your staff.
We conclude from the findings of this inspection that certain weaknesses exist
with respect to your administrative control of manually operated valves.
Specifically, the ability to more positively identify the position of a manual
valve, and the methods utilized to independently verify the correct position of
such valves, require your management attention. Both ESW system loops were
available, and apparently capable of delivering sufficient design flow to the
required essential equipment, so that the impact of these instances on system
operability and reactor safety was minimal. However, even though both instances
were found and corrected by your operations staff, we are concerned that the
valves in question remained' undetected for an indeterminant time in a condition
other than.the required locked and fully open position.
In your response to
Appendix A you are requested to provide a description of your actions taken and
planned relative to the adminstrative control of manually operated valves.
A violation of NRC requirements is cited in Appendix A and categorized under
the NRC Enforcement Policy, 10 CFR 2 Appendix C (49 FR 8583), March 8, 1984.
A reply is required and should be prepared in accordance with Appendix A.
Your reply to Appendix A is exempt from the Office of Management and Budget's
clearance procedures under the Paperwork Reduction Act of 1980, PL 96-511,
