101.0 QUALITY ASSURANCE REQUIREMENTS This program describes the quality assurance requirements for transport of sealed sources authcrized by material license 13-16347-01 and owned by Calumet Testing Services, Incorporated (CTS). The sealed sources are used for industrial radiography.
103.0 QUALITY ASSURANCE ORGANIZATION The Radiation Safety Officer (RSO) is responsible to the President for implementing the Transportation Quality Assurance Program. The RSO has the responsibility to identify quality problems, provide solutions to quality problems, verify implementation of solutions and stop work when necessary.
105.0 QUALITY ASSURANCE PROGRAM 105.1 This program applies to transportation of the following radioactive materials packages:
USA /6717/B(U)
For transporting Gamma Industries Model Century S exposure devices and Gamma Industries Model C-10 source changers (these devices may contain up to 100 curies Iridium-192)
USA /9036/B For transporting Source Production and Equipment Company Model C-1 scurce changer (this device may contain up to 100 curies Iridium-192)
USA /9127/8 For transporting up to 100 curies Cobalt-60.
This is a Gamma industries Model Gammatron 100A exposure device.
105.2 Personnel who perform activities described in this program as tre:ned to do so. Training is documented.
105.3 The RSO reviews the status and adequacy of the Transportation Quality Assurance Program on an annual basis.

133.0 CORRECTIVE ACTION 133.1 Defective packages type USA /6717/B(U) and USA /9036/B are either scrapped or returned to the vendor for disposition 133.2 Defective packages type USA /9127/B(U) are also exposure devices.
Disposition of defective transport packages of this type is determined by the RSO in accordance with procedures determined by the manufacturer.
133.3 In any case, corrective action is documented on the Nonconformance Report 135.0 QUALITY ASSURANCE RECORDS 135.1 The RSO maintains the following quality assurance records
- Records of use
- Records of reviews
- Records of audits
- Records of auditor qualification 135.2 Records are identifiable and retrievable 135.3 Records are retained indefinitely 137.0 AUDITS 137.1 The Transportation Quality Assurance Program is audited annually in accordance with written checklists (Exhibit 4).
137.2 Audits are conducted by trained personnel who do not have direct responsibilities in the areas being audited 137.3 Audit results are documented and reviewed by the President 137.4 Follow-up action is taken where indicated o
