Gentlemen:
This letter is in response to the Notice of Violation received as a

result of inspections conducted at Fort St. Vrain during the period
November 16 through December 31, 1986. The following response to the
item contained in the Notice of Violation is hereby submitted:
Failure to Wear Picture Badge Identification Within the Protected
Area
10'CFR Part 73.55(d),
Access
Requirements,
(5)
states,
"A
numbered picture badge identification system shall be used for
all
individuals who are authorized access to protected areas
without escorts.
Badges shall be displayed by all
.
.
.
individuals while inside the protected area." This requirement
is implemented by the licensee's Physical Security Plan.
Contrary to the above, on December 22, 1986, an on duty reactor
operator in the control room, a vital area within the protected
area, was not wearing a picture identification badge.
This
is
a
Severity
Level
IV
Violation.

(1) The reason for the violation if admitted:
'
Upon entering the protected area the duty reactor operator was
issued his personal numbered' picture badge identification.
The

picture badge incorporates the card needed to operate the card
reader security access system at Fort St.
Vrain.
The operator
attiched the badge to his outer jacket and proceded'to the
-
control room. The badge was removed and used to operate the card
,
reader to gain access to the control room vital area. After
-
entering the control
room the badge was reattached to the
,
operator's outer jacket.
The operator then removed his jacket
and. stored it, with the badge still attached,
in his personal
locker which is also located within the control room vital area!
The ' operator began his normal duties without displayingLhis badge
'
as 9 required by station security access procedures.
It should be
noted that the operator would not have been able to leave or
i
re-enter the control room without noticing that the. badge was not
on his person.
(2) The corrective steps which have been taken and the results
achieved:
'
The Station Manager and the Superintendent of Operations met with
the duty reactor operator involved in this incident.
The
importance cf complying with station security access requirements
was
re'smo5asized.
A
documented
verbal
reprimand
was
administerec to the reactor operator. The operator indicated
that the incident would not be repeated.
Following
the
reprimand, the operator contacted the site Senior Resident
Inspector for follow-up discussion of the incident.
(3) Corrective steps which will be taken to avoid further violations:

The Superintendent of Operations reviewed the incident with the
-
Shift Supervisors in the course of his weekly departmental
staff
meetings.
The importance of complying with station security
,
access requirements was re-emphasized.
Shift Supervisors were
instructed to review the incident and station security access
procedures with their respective crews.
(4) The date when full compliance will be achieved:
Full compliance was achieved on January 1, 1987.
