EXAMINATION REPORT NO. 50-245/86-21 (OL)
The week of December 12-19, 1986, the NRC administered examinations to employ-
ees of your company who had applied for licenses to operate your Millstone
Unit 1 facility. At the conclusion of the examination, the examination
questions and preliminary findings were discussed with those members of your
staff identified in the enclosed-report.
Two areas of concern found during the examination period warrant your immediate
attention. These are the accuracy of the simulator cause and malfunction book
and the large number of errors in the facility lesson plans. Details of these
concerns are in the attached report.
In accordance with 10 CFR 2.790 of the Commissions regulations, a copy of this
letter and the enclosure (s) will be placed in the NRC Public Document Room.
