PRESSURIZER RELIEF VALVE FAILURE DESCRIPTION:

WHILE PERFORMING 4.0.5-V-MISC., VALVE FAILED TO GIVE CONTROL PANEL AN OPEN INDICATION.

CORRECTIVE ACTION:

INVESTIGATED AND FOUND AIR LEAKING AROUND DIAPHRAGM.

TIGHTENED ALL BOLTS AND HAD CONTROL ROOM OPEN VALVE.

VERIFIED NO LEAKS AND VALVE FULL OPENED.

VERIFIED OPEN INDICATION IN CONTROL ROOM.

SALEM GENERATING STATION MONTHLY OPERATING

SUMMARY

- UNIT NO. 1 JULY 1986 SALEM NO. 1 The Unit began the period operating at 90% power.

On 7/10/86, #11 Condensate Pump was placed in service and at 2208 hours0.0256 days <br />0.613 hours <br />0.00365 weeks <br />8.40144e-4 months <br />, Unit load increase to 100% was initiated.

The pump is receiving power from Unit 2 2H Group Bus until the return to service of the #1 Auxiliary Power Transformer.

On 7/19/86 at 2100 hours0.0243 days <br />0.583 hours <br />0.00347 weeks <br />7.9905e-4 months <br />, Unit load was reduced to 90% in order to repair the mechanical seal of #11 Condensate Pump.

With the completion of repairs, the Unit was returned to full power operation on 7/20/86 at 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />.

On 7/21/86 at 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />, a controlled shutdown to Mode 3 (Hot Standby) was initiated to repair hydrogen leakage from the Main Generator.

Investigation revealed that the leakage was from the equalizing line (cracked pipe) between the two stator water cooling manifolds.

With the completion of Main Generator repairs Unit startup commenced on 7/28/86 at 0058 hours6.712963e-4 days <br />0.0161 hours <br />9.589947e-5 weeks <br />2.2069e-5 months <br /> and at 0649 hours0.00751 days <br />0.18 hours <br />0.00107 weeks <br />2.469445e-4 months <br /> the Unit was synchronized and brought to full power operation where it remained for the rest of the period.
