Based on the results of an NRC inspection conducted on September 12-16, 1988, i
j deviations from your comitments to the NRC were identified. The deviations i
i consisted of failure to :omplete an engineering guide and to fully reradiograph j
l the repair welds made to the emergency feedwater tank.
In accordance with the i
"General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1988), the deviations are ' listed below:

In a response dated June 30, 1987, to Violation 285/8713-01, concerning 1
failure to qualify a weld procedure, the licensee comitted to the f
1 completion of an engineering guide for preparation of welding procedures,
]
and procedure review by January 1988.

i In deviation from the above, the engineering guide has been prepared and j
i designated as Engineering (Manual CEl-6; however, as of September 16, 1988, i
l it has not been issued.

B.
In a response dated June 29, 1987, to Violation 285/8714-01, concerning the radiographic technique used on two of the emergency feedwater tank welds, the licensee stated that the reradiography of emergency feedwater j
tank girth welds G1 and G2 using source side penetrameters was complete.
In deviation from the above, reradiography of girth welds G1 and G2 was I
established during this inspection to be incomplete as a result of the l
radiographic film not encompassing all repaired areas.

Omaha Public Power District is hereby requested to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice.
This reply, should include for each deviation:

(1) the reason for the deviation if admitted, (2) the corrective steps that

bave been taken and the results achieved. (3) the corrective steps that will be i
taken to avoid further deviations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
