SPECIAL REPORT 85-06 SEQUOYAH NUCLEAR PLANT UNIT 1 Event Description and Probable Cause The biological shield blocks have been removed, and the unit 1 building equip-ment hatch opened for a refueling outage. This condition has been identified as a breach of a fire barrier penetration and will remain nonfunctional for the duration of the outage.

Corrective Actions Proper procedures for breaching fire barrier penetrations are being followed which include verification of fire detector operability and establishment of an hourly fire watch as required by the action statement of LCO 3.7.12.

The fire barrier will be restored at the completion of the outage.
