Document Control Desk March 25,1999 During the Unit 2 Cycle 9 midcycle outage, Southern California Edison (SCE) completed an inspection of steam generator tubes at San Onofre Nuclear Generating Station, Unit 2. In References 1,2, and 3, SCE reported that 100% of the tubes in Steam Generator E089 were examined by bobbin probe. During the Unit 2 Cycle 10 outage, SCE discovered the recorded data from the cycle 9 midcycle outage for 21 tubes was incorrect.
SCE now believes these tubes were probably not examined by bobbin during the Unit 2 Cycle 9 midcycle inspection. All 21 tubes were verified as no detectable discontinuity (NDD) during the Unit 2 Cycle 10 inspection (Reference 4).
SCE is evaluating the cause of this error and will incorporate the findings into the applicable inn oction processes. Enclosed is a table of the 21 tube locations in Steam Generator E089 in question.
No similar instances were discovered for Unit 2 Steam Generator E088.
