50-458 River Bend Station License: NPF-47 During an NRC inspection conducted on August 1 through September 17, 1988, a violation of NRC requirements was identified.
The violation involved a failure to follow a surveillance test procedure.
In accordance with the "General i
Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, l
. Appendix C (1988), the violation is listed below:
Failure to Follow Surveillance Test Procedure River Bend Station Technical Specifications, paragraph 6.8.1.a requires j
that procedures for activities identified in Appendix A of Regulatory
}
Guide 1.33, Revision 2, February 1978, be established, implemented and j
maintained.
Paragraph 1.d to Regulatory Guide 1.33 requires that administrative controls for procedural adherence be covered by written 4
procedure.
Administrative Procedure ADM-0003, "Development, Control and Use of Procedures," Revision 14, paragraph 6.5, requires in part, that procedures shall be present and referred to wnile the task is being performed for extensive or complex jobs where reliance on memory cannot be trusted.
Unless otherwise specifled, steps in a procedure snall be performed sequentially.
Contrary to the above, on August 15, 1988, at 5:00 a.m. (CDT) an engineered safety features actuation of the reactor water cleanup (RWCU) system occurred during the performance of Surveillance Test Procedure j
STP-058-4501, "Containment and Drywell Manual Isolation Actuation Monthly i
Channel functional Test." The instrumentation and control (I&C) technicians performing this STP failed to complete testing of the "B" i
channel of the manual Nuclear Steam Supply Shutoff System (NSSSS) prior to l
beginning testing on the "C" channel of the NSSSS.
This STP provides 1
a sequential steps which requires the I&C technician to complete testing of I
i the "B" channel prior to proceeding with the "C" channel.

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, i
