ERRORS IN OPERATOR LICENSE APPLICATIONS The operator license applications for the first group of operator and senior operator license candidates for Plant Vogtle and the associated certificates of medical history and examination were forwarded to the NRC by Georgia Power Company letter from Mr. James P. O'Reilly dated April 14, 1986. The Vogtle training staff subsequently informed this office of minor changes desired on the applicatier.s.

Recently, an extensive listing of application corrections and modifications was forwarded to this office by letter of Mr. L. T. Gucwa dated June 20, 1986.

NRC is concerned that the. numerous errors in the original applications may be indicative of an inattention to detail in their completion and an inadequate review prior to submission.

Furthermore, after having completed NRC medical review, two of the certificates of medical history and examination associated with the applications were found to have had their second pages transposed. The candidates' names do not appear on the second pages as the form is originally printed as one sheet; front and back, pages one and two, respectively. The error could possibly have gone undetected had not an abnormal physical condition with one of the candidates led to a request for additional information. NRC is concerned that additional Form NRC 396, Certificate of Medical History, errors may have gone unnoticed and may occur in the future.

We request your attention be brought to this problem to reduce errors in future applications. We request your assurance that all applications have been reviewed carefully and that no further errors exist. We appreciate your cooperation on this matter.
