NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-373 As a result of the inspection conducted on June 11 through July 21, 1986, and in accordance with the General Policy and Procedure for NRC Enforcement Action (10 CFR Part 2, Appendix C), the following violations were identified:

1. Technical Specification 6.2.A.6 requires that detailed written procedures be prepared, approved, and adhered to for preventive and corrective maintenance operations which could have an effect on the safety of the facility. Licensee procedure LOP-RP-04 requires the installation of a jumper to prevent a Reactor Building Ventilation System isolation while transferring the reactor protection bus power supply.

Contrary to the above, on June 13, 1986, Procedure LOP-RP-04 was found to be inadequate as it prescribed the wrong termination points for the jumper, resulting in a Reactor Building Ventilation System isolation.

This is a Severity Level IV violation (Supplement I).

2. Technical Specification 6.2.A.7, requires detailed written procedures for surveillance and testing requirements to be prepared, approved, and adhered to. Procedure LES-PC-107, " Unit 1 Group 7 Isolation Logic Systems Functional Test", Step F.3.d requires switch IB21H-S19D to be placed in test, to test the Transverse Incore Probe (TIP) system logic.

Contrary to the above, on June 13, 1986 the Unit 1 operator did not place .

switch 1B21H-S19D to the test position while performing LES-PC-107.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violation; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
