Inspection No. 50-289/88-17

This letter refers to the special safety team inspection conducted i.y Mr. G.

Napuda of this office on August 8-19, 1938 at the Three Mile Island Nuclear

Generating Station, Middletown, Pennsylvania of activities authorized by NRC

License No. DPR-50. Mr. Napuda discussed the results of the inspection with

you and other members of your staff at the conclusion of the inspection and

during an October 4,1988 telephone conversation with Mr. T. G. Broughton of

your staff.

Areas examined during this inspection are described in the NRC Region !

Inspection Report which is enclosed with this letter. Within these areas, the

inspection consisted of selected examinations of procedures and represer.tative

records, interviews with personnel, and observations by the inspectors. The

team concluded that the overall outage activities were conducted in a

technically competent fashion except as hereafter noted.

Based on the results of this inspection and 9ur discussion on August 24, 1988

at the NRC Region I office, it appears that certain activities were not

conducted in full complience with NRC requirements, as set forth in the Notice

of Violation, enclosed herewith as Appendix A.

Two violations were identified

relating to (1) inadequate procedures and (2) failure to review a procedure

change within the Technical Specifications allotted time period and the

establishment of a procedure that pertatts this practice.

These violations have

been categorized by severity level in accordance with the "General Statement

of Policy for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement

Policy). You are required to respond to this letter and in preparing your

response, you should follow the instructions in Appendix A.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
