c APPENDIX A General Electric Company Nuclear Energy Business Operations Docket No. 99900403/82-03 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on June 28-July 1, and July 13-15, 1982, it appears that certain of your activities were not conducted in accordance with your commitments, as indicated below:
Section 5 of Topical Report No. NEDO-11209-04A, Revision 3, dated August 1981, states in part, " Documented instructions, procedures, and drawings are utilized to communicate quality requirements throughout all phases of design, purchasing, manufacturing, and construction.
Activities affecting quality, including methods of complying with 10 CFR Part 50, Appendix B, are delineated, accomplished, and controlled by such documents as policies, procedures, operating instructions, design specifications, shop drawings, planning sheets, test and inspection procedures, and standing instructions."
Contrary to the above, documented instructions or ' procedures were not utilized 1
to communicate quality requirements relating to the dispositioning of potential design action items.
These potential design deficiencies were dispositioned without documented criteria or clearly established responsibilities for the actions.
For example, 16 of 18 potential containment design issues placed on the Design Action List in January 1982 were dispositioned as requiring no further work by General Electric without the process or evaluation criteria being documented.
