o U.S. Nuclear Regulatory Commission License Management Branch Division of Fuel Cycle and Material Safety Washington, D.C. 20555 hE License Number SNM-1581 We hereby request renewal of the above captioned special nuclear material license which covers the implantation of nuclear-powered cardiac pacemakers in humans.
We request the same possession limit.
There have been no changes in personnel from those presently licensed.
To date we have implanted 2 patients.
There have been no adverse reactions or malfunctions of the pacemaker system or leads.
Both patients are siill alive and continue under the care of Dr. Robert W. Driscoll who did the implants.
Each patient has been instructed regarding possible future replacement of the pacemaker and the recovery of the pacemaker in case of death.
This application is intended to supplement previous correspondence with your office as outlined in condition #16 of the present license.
We understand that there is no fee for this renewal.
