NPPD Response to IE Inspection Report 50-298/78-17

Dear Mr. Seyfrit:

This letter is written in response to the letter dated October 30, 1978, transmitting IE Inspection Report 50-298/78-17 which indicated that certain of our activities were not conducted in full compliance with NRC requirements.

Statement of Infraction A 4

Criterion V of Appendix B to 10 CFR 50 requires activities af fecting quality be prescribed by documented instructions, procedures or drawings and be accomplished in accordance with these instructions, procedures or drawings.

This requirement is amplified in Section 2.5 of the licensee's approved Quality Assurance Manual.

contrary to the above requirements, Procedure 1.4, " Station Rules of Practice," was not adhered to in that Standby Liquid Control System Valve, SLC-10, was not sealed in the locked open position, nor had a note been made in the Shift Supervinor's Log as to this condition as required by the above procedure.

This is an infraction.

Discussion and Corrective Action Taken on 9-25-78, during per formance of surveillance procedure 6. 3.8.2, SLC Pump Operability Test, sealed open valve SLC-10 (SLC tank outlet) was closed as per the procedure and the seal removed.

The Scation Operator performing the test laid the seal on the floor intending to return it to t.he Shift Supervisor upon completion of the surveillance procedure.

Surveillance procedure 6.3.8.2 includes a step requiring sign of f for reopening SLC-10 and restoring the system to a normal lineup.

SLC-10 was reopened and the system returned to normal lineup with the exception that a new seal was not placed on SLC-10, 781205 0 0 S 2

A*new seal number 0688 was issued and placed on SLC-10 securing it in the open position at the time it was brought to the Shift Supe rviso r's attention.

This personnel error was discussed with the personnel involved.

Corrective Action to be Taken to Avoid Further Noncomo11ance A copy of Administrative Procedure 1.4.4.1.7, Valve Seals Log, has been routed to all Operations personnel for their review.

Additionally, the four surveillance tests performed for the SLC system which require operation of valve SLC-10 have been revised to ensure a new ceal is placed on valve SLC-10 upon completion of the test.

Date When Full Compliance Will Be Achieved Full compliance has been achieved as of November 20, 1978.
