SPECIAL REPORT 86-04 SEQUOYAH NUCLEAR PLANT UNIT 2 Event Description and Probable Cause The biological shield blocks have been removed, and the Unit 2 Building equipment hatch opened for fire protection modifications in the annulus.
This condition has been identified as a breach of a fire barrier penetration and will remain nonfunctional for the duration of the outage.
Corrective Actions Proper procedures for breaching fire barrier penetrations are being followed which include verification of fire detector operability and establishment of an hourly fire watch as required by the section statement of Limiting Condition for Operation (LCO) 3.7.12.
The fire barrier will be restored at the completiur(of the outage.
