Dear Mr. Martin:

The Sacramento Municipal Utility District hereby submits Special Report No.

86-14 in accordance with Technical Specification Sections 3.14.4.2 and 6.9.5.E.

During cold shutdown on July 23, 1986, the Zone 78 Carbon Dioxide (C0 ) fire Room (NSEB) protection system for the Electrical Equipment Room and Battery 2

was out of service for more than 14 days.

The correct fire watch has been present since July 23, in accordance with Technical Specification 3.14.4.2.

The Technical Specifications require that after a C02 system is out of service for more than 14 days, a special report be submitted pursuant to Technical Specification 6.9.5.E within 30 days.

CAUSE OF THE INOPERABILITY On July 23, 1986, Fire Damper FD-34N did not meet the acceptance criteria of Surveillance Procedure SP.201.03M.

Because of that deficiency, the fire damper was determined to be inoperable. The fire damper provides isolation i

for a CO2 zone so personnel in adjoining areas are not overcome by C02 gas.

The Shift Supervisor on duty judged this inoperable condition to be a personnel safety hazard and tagged-out the Zone 78 C02 system.

On July 30, 1986, the District determined that HVAC Damper HV-50161 provided sufficient redundancy to FD-34N, allowing the Zone 78 CO2 system to be returned to service.

However, the backup battery to the Zone 78 C02 panel drained during the time the system was tagged-out.

The Zone 78 CO2 system remained out-of-service because it was not battery backed-up.

7 I.b J. B. Martin September 4, 1986 PLANS AND SCHEDULES FOR RESTORING THE SYSTEM TO OPERABLE STATUS An hourly fire watch has been posted for Zone 78 since July 23, 1986.

The Zone 78 CO2 system is currently powered by the normal off-site source.

Reconnecting the battery to the off-site power source recharged the back-up battery. The Zone 78 CO2 was declared operational on August 19, 1986.
