SOUTH TEXAS PROJECT ELECTRIC GENERATING STATION N O. b 0 P-9.040 0 OPERATIONS ENGINEERING PROCEDURE j f(i OF PERSONNEL CERTIFICATION PROCEDURE FOR VISUAL EXAMINATION PAGE 3 47 PER ASME B&PV CDDE, SECTION XI EFFECTIVE [ DATE 7_1.gg j 1.0 PURPOSE 1.1 To establish guidelines for the indoctrination, training, y examination, qualification, and certification of visual ~ examination personnel whose specific function requires knowledge appropriate to the principles and applications of visual examinations they are to perform, monitor, witness or evaluate. l 2.0. SCOPE t 2.1 These guidelines may be applicable to personnel who monitor, witness or evaluate Pre Service Inspection (PSI) and Inservice Inspection (ISI) visual examinations by the VT-1, VT-2, VT-3, or VT-4 methods as defined in the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel (B&PV) Code, Section XI. 2.2 These guidelines are applicable to HL&P employees and contracted personnel who are assigned to perform visual examinations required by Section XI under the technical direction or supervision of HL&P Operations Support Engineering Group (OSEG). OSEG or its ('<, representatives shall determine specific applicability of visual examinations performed on behalf of OSEG. In addition, OSEG may choose to implement or impose this procedure on other visual examination activities not prescribed by Section XI. 2.3 The requirements for qualification of visual examination personnel, as outlined in Section XI, Paragraph IWA-2300 were adopted as the basis for this procedure. This procedure meets the applicable requirements of Section XI, 1980 Edition with addenda through the Winter 1981. 3.0 DEFINITIONS Significant words and phrases used in this procedure are defined below. Other words with unique meanings or usage are defined in SNT-TC-1A, ANSI N45.2.10, " Quality Assurance Terms and Definitions", or Section XI. 3.1 Visual Examination - As used herein, refers to general observation using the human eye, low power magnification (10X or less), or other optical aids as necessary to perform adequate examinations. During PSI or ISI, the term refers specifically to VT-1, VT-2, VT-3, or VI-4 types of visual examinations as used and define ~d in Section XI. ( S3/ PROC /A

OEP-9.04Q 0 OPERATIONS ENGINEERING PROCEDURE T' PERSONNEL CERTIFICATION PROCEDURE FOR VISUAL EXAMINATION PAGE 4 OF 47 ) PER ASME B&PV C;0DE, SECTION XI EFFECTIVE DATE 7-1-86 3.2 Indoctrination - As used herein, refers to the method used by OSEG to assure that contracted visual examiners or HL&P personnel certified to some other written certification procedure are also qualified to this procedure. The prospective visual examiner shall read the appropriate OSEG visual examination procedures and pass the Specific tests for applicable methods to assure comprehension of the OSEG requirements. 3.3 Training - As used herein, refers to the program and methods developed to impart knowledge and skills necessary for qualification. Training methods may include formal lecture, demonstration, self-study, audiovisual aids, on-the-job training, etc. The actual training used to qualify personnel may include one or more of the listed methods. 3.4 Qualification - As used herein, refers to education, personnel testing and evaluation, skills, training and experience required for personnel to properly perform the duties of a specific job. 3.5 Personnel Evaluation - As used herein, refers to the initial review and analysis performed by the VT Level III prior to his j] certification of personnel or to the review and analysis >j performed by the VT Level III prior to his recertification of personnel. In either case, this evaluation is based upon 4 qualification and satisfactory completion of any required training _ 3 and testing. 3.6 Testing - As used herein, refers to the physical, written or practical examinations or tests taken by the candidate for certification after training which are used to evaluate the candidate comprehension of training materials and/or his fitness for certification as defined below. The term also refers to tests or examinations to maintain current certifications or to demonstrate fitness for recertification. ? .i 3.7 Certification - As used herein, refers to written testimony of personnel quelification. 3.8 Certifying Agency - Traditionally refers to the employer of the personnel being certified. However, in the case of this certification practice, OSEG may certify contractor personnel who are not already certified by their employer. OSEG may require that certified contractor personnel be indoctrinated, examined and certified to this practice. i. 3.9 Employer - As used herein, refers to the corporate or private I entity which employs personnel for wages or salaries (i.e. either. l HL&P or the specific contractor for which the person works). ?_ 4 I s s3/ PROC /A r

[ 3.11 Interpretation - As used herein, refers to determining the identification of an indication against a set of recording criteria (e.g. "all cracks are to be recorded"), e t 3.12 Technical Evaluation - As used herein, refers to determining the suitability for an item or component for service or continued service. 1 3.13 Indication - As used herein, refers to relevant conditions, etc. defined in Section XI. 3.14 Written Instruction - A non mandatory, detailed instruction written for use by a Level I Examiner in performing a specific examination or test.

These responsibilities, or portions thereof, may be delegated to the Principal Engineer of OSEG or to others. 5.2 The Principal Engineer of OSEG shall assure that the requirements of this procedure are met for applicable activities within his division. 5.3 The Supervising Project Engineer responsible for the PSI /ISI program shall assure that the requirements of this procedure are met for applicable activities within his section. 5.4 The PSI /ISI Lead Engineer (or his designee) shall be responsible for: 5.4.1 Coordination of the development and implementation of training and testing programs for qualification of HL&P and '] contractor personnel. ) 5.4.2 Coordinator of the eye examinations required by this procedure for qualification of HL&P and contractor personnel. 5.4.3 Coordination of the establishment and providing technical supervisicn of contracts for Level III services required for implementation of this procedure. { 5.4.4 Coordination of the preparation of records required by this procedure and their transmittal to RMS for storage. 5.5 The HL&P Level III shall be responsible for those duties assigned .j to him (or his designee) by this procedure. VT-1: condition of Class 1 parts, components, or 3 surfaces ( b. VT-2: if leaks exist in pressurized systems or components f c. VT-3: general mechanical condition of components / f supports i d. VT-4: operability of components and component ( supports 6.2 Levels of Qualification As required by Section XI, there shall be three basic levels of qualification, i.e. Level I, Level II, and Level III. These r levels may be further subdivided by HL&P for specific situations l-where additional levels of skills and responsibilities are deemed (.. necessary. Additionally, limitations may need to be imposed on some certifications. Such limitations will be clearly noted on i' the individual's certification record (Attachment OEP-9.04Q-14) e.g., VT-3 certification for Component Support Inspection only, and further described in an addenda to this procedure. HL&P visual examination personnel may be certified directly to Level II, thus bypassing Level I certification. In such cases, the experience and training time (but not the testing) used to qualify the personnel shall consist of the sum of the times required for Level I plus that required for Level II in total for both training and experience as specified herein. 6.2.1 Trainee 6.2.1.1 Prior to certification to at least Level I, the candidate for certification is considered a trainee. As a trainee, he must work directly with a Level I, Level II, or Level III individual while he is accumulating enough experience for certification at the desired level (I or II as described in paragraph 6.2 above). L 6.2.3.2 Shall be qualified to set up and use optical aids or measuring devices. 6.2.3.3 Shall be qualified to interpret and evaluate results with respect to applicable instructions, procedures, codes, standards and specifications. 6.2.3.4 Shall be thoroughly familiar with the scope and limitations of the method (s) for which qualified and shall be capable of exercising assigned I responsibility for the on-the-job training and guidance of trainees and VT Level I personnel. l l 6.2.3.5 Shall be able to prepare written instructions, review procedures, and to organize and report the results of visual examinations. i )l S3/ PROC /A

('i PERSONNEL CERTIFICATION PROCEDURE FOR VISUAL EXAMINATION PAGE OF 9 47 PER ASME B&PV C,0DE, SECTION XI EFFECTIVE DATE 7-1-86 I 6.2.4 VT Level III i 6.2.4.1 Shall be capable of performing all the duties of a VT Level II. i 6.2.4.2 Shall be capable of establishing techniques; f interpreting codes, standards, specifications, and procedures; and designating the specific j visual method (s), technique (s), proceoure(s) to J be used. 6.2.4.3 Shall be responsible for the VT operation he is qualified for and assigned to; and shall be capable of interpreting and evaluating results in terms of existing codes, standards, and specifi-cations. 6.2.4.4 Shall be capable of planning and supervising visual examinations; preparing, reviewing, or approving procedures; evaluating the adequacy of (' VT activities required to accomplish objectives; (. and to assist the design engineer in establishing recording or acceptance criteria where none otherwise exist. He shall have general famil-iarity with other appropriate visual methods. 6.2.4.5 He shall be capable of training, examining and certifying visual examination personnel at any level, in accordance with this written practice. 6.3 Administration of Examinations 6.3.1 Examination of visual examination personnel shall be under the administration of a Level III in the applicable VT method. l-6.3.2 The actual administration and grading of examinations may l' be delegated in writing to a duly selected representative of the Level III (Attachment OEP-9.04Q-12). At the option of HL&P, Level III services may be purchased from an outside agency or contractor as described in paragraph 6.7 of this written practice. ( S3/ PROC /A

OEP-9.040 0 OPERATIONS ENGINEERING PROCEDURE ~ )'- PERSONNEL CERTIEICATION PROCEDURE FOR VISUAL EXAMINATION PAGE 10 OF 47 j PER ASME B&PV CdDE, SECTION XI EFFECTIVE DATE 7-1-86 6.4 Education, Training, and Experience 6.4.1 Personnel considered for certification in visual methods shall have sufficient education, training and experience (as defined herein) to ensure understanding of the principles and procedures for those methods of visual examination in which they are being considered for certification as demonstrated by the results of the required tests. 6.4.2 To be considered for certification, a candidate shall satisfy a minimum of one of the criteria listed in Attachments OEP-9.04Q-01 and OEP-9.04Q-02 of this procedure, for the applicable Level and VT method. Documented training and/or experience, as demonstrated on the individual's resume or supplemental experience records, gained in positions and activities equivalent to those of Level I, II or III, as judged by HL&P, prior to establishing this procedure or a certification program in general acccedance with SNT-TC-1A or ANSI N45.2.6 shall be considered as satisfying the requirements of this written practtee. w 6.4.3 The above listed personnel information may be obtained and documented on the " Visual Examination Pre-Training Questionaire", (Attachment OEP-9.04Q-15). 6.5 Training Programs 6.5.1 Training provided by HL&P for the various visual l examination methods should follow outlines similar to those provided in Attachments OEP-9.04Q-03, OEP-9.04Q-04, OEP-9.04Q-05, OEP-9.04Q-06, and OEP-9.04Q-07 of this procedure. As an option, HL&P may elect to send personnel j to receive formal training or have training contracted and presented at HL&P facilities. 6.5.1.1 The training may be administered by any of the methods or combination of methods described in paragraph 3.3 and so documented. 6.5.1.2 When the outlines attached to this procedure are j followed, the basic VT training course and examination is a prerequisite for certification tc any other VT method. OEP-9.04Q 0 A.*( A OPERATIONS ENGINEERING PROCEDURE j' ( PERSONNEL CERTIFICATION PROCEDURE FOR VISUAL EXAMINATION PAGE 11 OF 47 PER ASME B&PV CODE, SECTION XI EFFECTIVE DATE 7-1-86 6 i I 6.5.2 Other VT courses follo<ing different outlines or training scheme, which in the opinion of HL&P, meet the requirements of Section XI (Subsection IWA) and Section l 6.5 of this procedure, may also be presented by HL&P, outside training consultants or contractors to train, l qualify and test VT examiners working for, or on behalf of, HL&P. 6.5.2.1 In addition, HL&P or contracted personnel who have successfully completed the Visual Examin-ation training courses offered by the Electric Power Research Institute (EPRI) NDE center entitled, " Visual Examination Technology, Level I, II, and/or III", shall be considered by HL&P to have met the training requirements necessary for applicable levels or certification, including the total training hours for the basic VT training course and each separate VT method. 6.5.3 For each Visual Examination method applicable, information (- regarding relevent training, testing and retraining shall be documented for all examination personnel. This infor-mation and acceptance thereof shall be noted on the visual examination " Training Record", (Attachment OEP 9.04Q-09). 6.6 Testing Program (Examinations) 6.6.1 All levels of VT personnel shall be initially qualified by written tests. In addition, Level I, II, and III personnel shall be recertified by retraining and testing on a triennial basis. 6.6.2 The tests shall be designed to ensure the comprehension of the training material as demonstrated by the test results. The candidate for certification must also demonstrate his ability to pass the physical (vision acuity) requirements I established in this procedure. 6.6.3 A VT Level III or his selected representative (s) shall administer and grade tests for VT Level I, Level II and other Level III personnel. 6.6.4 VT personnel for all methods and levels shall be initially examined, and thereafter annually, by qualified personnel to ensure they have natural or corrected visual acuity in at least one eye as described below. Qualified personnel ( include the Level III, a Nurse or Eye Doctor. In addition, personnel trained to administer such eye examinations are considered qualified. S3/ PROC /A

The ability to perceive an Ortho-Rater No. 10 test pattern or other near distance test pattern equivalent to a Snellen fraction of 20/20 is also acceptable. 6.6.4.2 Far distance acuity as demonstrated by the ability to read the equivalent of the 20/30 line on the standard Linear Snellen test at 20 feet or other equivalent far distance test. 6.6.4.3 The capability to distinguish and differentiate the contrast between colors shall be verified by using standard color perception test; such as correctly reading 10 of 14 plates on the Pseudo Isochromatic test. The Level III shall consider the results of the color test when evaluating and certifying personnel and shall document the basis. for certification. 6.6.4.4 Results of visual acuity and color perception test shall be documented on Attachment OEP-9.04Q-08 and kept on file in accordance with Section 7.4. 6.6.5 Tests for Level I and II personnel shall be selected, devised or approved by a Level III for each applicable method and level and shall include the following: 6.6.5.1 The written general test covering the general examination principles relative to the applicable VT method. 6.6.5.2 The written specific test covering optical aids, examination procedures, and techniques the candidate may encounter in his specific assignment. The test shall also cover specifications or codes and recording or acceptance criteria used by HL&P. 6.6.5.3 A written practical test or documented practical demonstration (as described below) performed to the satisfaction of a Level III which establishes ) l4 that the candidate is familiar with the visual method (s) and can examine parts and analyze and report the resultant information. i S3/ PROC /A l ~ _ _ _ -... _. _ _

6.6.6 Level III examinations shall include the following: 6.6.6.1 Basic Examination (required only once when more than one method examination is taken)
- a. Twenty (20) questions relative to understanding SNT-TC-1A and its implementation.

- b. Fifteen (15) questions relative to applicable materials, fabrication and product technology.
- c. Fifteen (15) questions selected from or similar to those used in the Level II General Tests for VT-1, VT-2, VT-3, and VT-4.
6.6.6.2 Method Examination (one for each VT Method)
- a. Thirty (30) questions relating to fundamentals and principles selected or devised by the employer.
- b. Fifteen (15) questions relating to application and establishment of techniques and procedures which are selected or devised for each method by the employer.
- c. Twenty (20) questions relating to capability for interpreting codes, standards, and specifications relating to the VT inethod.
6.6.6.3 Specific Examinatior (one for each VT Method)
- a. Twenty (20) questions relating to specifications, equipment, techniques, and i.
procedures applicable to HL&P components, systems, or plants, to the VT method to be employed, and to the administration of HL&P's written practice. S3/ PROC /A
