Ig Comission regulations. Within five (5) days after determining that any source y
ll has leaked, the licensee shall file a report with the Material Licensing Branch, a
ji Division of Fuel Cycle and Material Safety, U. S. Nuclear Regulatory Commission, I
Il Washington, D. C. 20555, describing the source, the test results, the extent of I

contamination, the apparent or suspected cause of source failure, and the i
corrective action taken. A copy of the report shall be sent to the U. S. Nuclear j Regulatory Commission, Region I, 631 Park Avenue, King of Prussia, Pennsylvania, lg 19406.

The periodic leak test required by this condition does not apply to sealed
- I sources that are stored and not being used. The sources excepted from this test ji shall be tested for leakage priar to any(use or transfer to another person unle they have been leak tested within three 3) months prior to the date of use or y
transfer.

A.
Each encapsulated plutonium source designed for the purpose of emitting neutron i
l or gamma radiation shall be tested for leakage at intervals not to exceed six (6)
I months.
In the absence of a certificate from a transfer indicating that a test l
has been made within six (6) months prior to the transfer, a sealed source y
received from apother person shall not be put into use until tested.

packages the design of which the NRC has specifically approved for transport of I
