Dear Sir:
During the week of July 28, 1986 to August 1,1986, the NRC corn.ated an inspection of the equipment qualification program at McGuire Neclear Station.
The results of this inspection were presented by Mr. Steve Alexander (NRC/IE/VPB) at an exit interview on August 1, 1986.
Attached for the use of the NRC in preparation of the subject inspection report is a discussion of the identified findings and the committed Duke action in response to each item.
Duke requests that the information contained herein be reflected in the subject inspection report.

Unidentified wiring inside Limitorque valve operator limit switch compartments was found during site inspection.
BACIDROUND: Upon receipt of IE Notice 86-03, Duke contacted Limitorque to determine what type of wire had been supplied to Duke on a Shop Order basis.
Duke evaluated each valve on the basis of the Limitorque letter stating what type of wire should have been provided.
Based on the Limitorque information and Duke procedures for field installed wire, Duke had concluded the valves were qualified.
However, based on the inspection unidentified wire had been supplied with the valve operators.
DUKE ACTION: Duke committed to inspect and repair as necessary all McGuire Unit 1 safety-related Limitorque. valve operator required to operated in harsh environment.
The inspections and necessary repairs will be completed prior to Unit 1 re-start.
Subseq uently, the issue has been evaluated and discussed with NRC Region 2 for all other Duke Nuclear Units.
Appropriate station action and any necessary J00's have been established and agreed upon by region personnel.

Shipping dust caps had not been renoved on grease pressure relief port on some Limitorque valve operators.
BACEDROUND: Duke provided to the NRC prior to the Exit Interview an evaluation justifying that the shipping caps would not impair the ability of the valves to perform their required safety function during the postulated accident.
DUKE ACTION: The evaluation has been formalized and included in Duke files.
Work requests were written to assure all dust caps are removed.
A specific instruction will be added to the Limitorque Installation / Instruction Manual to require removal of dust caps at the time operators are wired for power.
3 Penetration qualification files did not include a functional performance evaluation to address potential circuit leakage currents during postulated accidents.
McGuire Nuclear Station, Unit 1 NRC Exit Interview August 14, 1986 Page 2 BACMHWND: Duke had analyzed worst case instrtmentation circuits for effects of leakage currents on required instrtment accuracy.
The analyses regarding penetration leakage currents were shown to the NRC a udi tor.
In that files on cable leakage effects could not be found, analyses were re-generated prior to the exit interview and also shown to the NRC auditor.
The. NRC auditor was satisfied with the technical content of the analyses.
DU B ACTION: Duke will include appropriate references in the penetration qualification files to files containing the circuit analyses for potential leakage current effects on instrtmentation accuracy.

Qualification file for Samual Moore PVC cable did not support qualification.
BACEROUND: A similarity analysis to qualified cable was contained in the qualification file.
It is Duke's position that the analysis was adequate and that the NRC auditor's conclusions are based on a j
difference in technical opinion on what is an acceptable analysis.
I DU E ACTION: Duke committed to revise the qualification file to show more conservatian based on the McGuire :: pacific application of the cable.
A draf t of the revision was provided to the NRC auditors p'ior to the Exit Interview.
The auditors found the revision acceptable.
5 Barton 386A transmitter qualification file did not adequately justify i
all test anomolies, i
BACEROUND: The qualification file did address test anomolies.
It is Duke's position that the NRC auditor's findings are based on a difference in technical opinion on what is an acceptable assessment of f
these anomolies.
l DU H ACTION: Duke provided the NRC auditors with additional technical information justifying the McGuire application of the Barton 386A.
Duke also committed to augment the qualific_ tion file with a similarity analysis to the Barton 764 transmitter.
A draf t of the analysis was shown to the NRC auditors prior to the Exit Interview.
The auditors felt the file additions were acceptable.

Rockbestos high-temperature silicon rubber cable qualification file not sufficient to support qualification.
BACEROUND: Duke action to IE Notice 84-44 has been to follow the Rockbestos retest program.
To date, all retest results have validated the original test conclusions.
Duke considers the original test conclusions of the high-temperature silicon rubber mble to be valid.
DUIE ACfION: Duke presented the inspection team with supplanental documentation to show the substantial conservatism for the Mdluire applica tion.
This documentation will be filed with the applicable Rockbestors' qualification file.
Upon completion of remaining Rockbestos retests, applicable files will be supplemented with the test repor ts.
7 Raychem splice applications which do not conform to bend radius recommenda tions.
BACEROUND: Duke presented to the inspection team documentation concerning Raychem splices.
The results of a Duke inspection of splices and the station installation procedure were part of this do cumentation.
All problems identified during the Duko inspections -
were documented and satisf actorily resolved.
The Duke installation procedure adequately addressed all concerns of IE Notice 86-53 (i.e.,
overlap, sizing, not bending until cool to the touch, etc.) with the exception of bend radius recommendations.
Subsequently, the inspection team was presented with a letter from Raychem detailing recommendations for splices that had a tight bend radius.
Therefore it is Duke's position that the McGuire Raychem splicee are qualified for their application.
IIIII ACTIOEt Duke committed to continue to follow Raychem recommendations for any splices which have a small bend radius.
- Also, Duke plans to follow industry testing which is intended to provide additional evidenoe that amall bend radius Raychan splices are qualified.
