On June 27, 1986 at 2315 the Loose Parts Monitoring System was declared inoperable. At that time Technical Specification Action Statement 3.3.7.9.a was entered which allows thirty days to repair the malfunction. During that period Public Service Electric & Gas worked with the vendor Rockwell International to troubleshoot and repair the inoperable channels. Although the vendor stated that it was usual to experience difficulties at this stage of operations, the erroneous information for calibration and tuning of the system together with out-of-spec components supplied by Rockwell hindered troubleshooting efforts. As a result, the channels were not repaired in the thirty days allowed by the Technical Specifica-tions. The cause for the system inoperability was due to an ,

inadequate calibration. The vendor installed new amplifier cards and system operability will be proven through the successful completion of surveillance tests. Presently all twelve channels have been successfully calibrated and the system is expected to be declared operational by the end of the week. This Special Report is being submitted in accordance with Technical Specifications 3.3.7.8 and 6.9.2.

DACEGACUED During December of 1985 the Loose Parts Monitoring System was successfully preoperationally tested with the exception of a printer which required circuit modification. Rockwell International, the system's vendor, was present during this testing. Although the preop was completed satisfactorily, it must be noted that the tests were performed when the system was not filled with water. As a result, with the reactor systems filled and the reactor critical, several loose parts alarm channels activated. The system was declared inoperable on June 27, 1986 and Technical Specification Action Statement 3.3.7.8.a was entered.

The statement allows thirty days to repair the malfunction and declare the system OPERABLE. The action statement specifies "With one or more loose-part detection system channels inoperable for more than 30 days, prepare and submit a Special Report to the Commission...".

DESCRIPTIDE DE TBE EVENT The vendor arrived onsite to install a printer modification and to correct the alarming condition of the Loose Parts Monitoring System channels. The circuit modification to the printer corrected the problem identified during preoperational testing. The alarm circuits were modified at the direction of the vendor, but the problem persisted. Under vendor direction amplifier cards were then modified. This, however, did not correct the problem and the vendor returned to his office for an intensive research and development erfort. By this time, the thirty days ha.d passed and the Loose Parts Monitoring System remained inoperable. On July 27, 1986, the thirty days had elapsed and a Special Report was required.

ANALYSIS DE TEE EVENT Since the preset configuration of the system was determined when the reactor systems were not filled with water and conditions were not indicative of an operating plant, the alarm setpoints and the calibrations would have to be changed when operations began. The vendor stated it was common for the system to experience difficulties at this stage of operations. In this case, the field signal strength was too high for the preset configuration of the amplifier cards. This produced false alarms indicating loose g,,o M 2 .

In addition circuit board modifications were all performed under the vendor's direction and still failed to produce satisfactory results. Troubleshooting activities were also complicated by a change in the calibration tools. It was realized that a new type instrument was required in order to calibrate the channels properly. Although there was a diligent effort to repair the Loose Parts Monitoring SysF 7 within the required time frame, the described difficulties preve.ited the system from being declared OPERABLE. As a result, the vendor returned to his office to resolve the amplifier card problem and develop new calibration tools.

CORRECTIVE ACTIDBE Corrective actions include the installation of new amplifier cards, the development of instruments to be used in channel calibrations, and the performance of surveillance tests to prove the system OPERABLE. The vendor provided new amplifier cards and they were installed at his direction. The new tools were utilized in the channel calibrations. To date all twelve channele have been calibrated successfully and the system is expected to be declared operational by the end of the week.

This event posed no threat to the public health and safety and this report is being submitted pursuant to the requirements of Technical Specificationi 3.3.7.8 and 6.9.2.
