APPENDIX A NOTICE OF VIOLATION Rockwell International Corporation Rocketdyne Division Atomics International 6633 Canoga Avenue Canoga Park, California 91304 As a result of the inspection conducted July 7-10, 1986 and in accordance with the NRC Enforcement Policy, 10 CFR Part 2, Appendix C, the following violations were identified:

A. License Condition 9 states, in part, that licensed material is authorized for use in accordance with statements, representations, and conditions contained in Part I of the licensee's application dated August 20, 1982.

The " Health and Safety Sections for Renewal Application of the Special Nuclear Materials License SNM-21, Docket 70-25, issued to Energy Systems Group of Rockwell International" ESG-82-33 was submitted with the licensee's application.

1. Section 11.6.4 of ESG-82-33 states, in part, that refresher training in radiation safety is 'given at two year intervals.

Contrary to the above requirement, an employee received basic training in radiation safety on May 23, 1980 and worked as a radiation worker from June through July 1982, December 1982 through March 1983, and October,through December 1983, without having taken refresher training. The employee subsequently received refresher training in radiation safety on March 7,1984, but again has been working as a radiation worker since June 21, 1986 to date without having taken another refresher training at the stated two year interval.

This is a Severity Level IV Violation (Supplement VI).

2. Section 13.5.1 of ESG-82-33 states, in part, that the normal frequency for surface contamination surveys in areas in which work is conducted with unencapsulated radioactive materials is once per week Section 12.5 states, in part, that the results of surface contamination surveys are maintained permanently.

Contrary to the above requirement, records hac not been maintained of the weekly contamination surveys conducted in the Hot Cell Facility (Building 020) between May 19 and June 5, 1986.

This is a Severity Level V Violation (Supplement VI).

B. 10 CFR 20.201(b) requires that each licensee shall make or cause to be made such surveys or evaluations as may be necessary for the licensee to comply with the regulations in this part, and (2) are reasonable under the circumstances to evaluate the extent of radiation hazards that may be 8608050214 860731 PDR ADOCK 07000025 C PDR

, ; ~. , present. 10 CFR 20.401(b) requires, in part, that the licensee shall maintain records of such surveys or evaluations.

Contrary to the above requirements at the time of the inspection, the

.. evaluations made by'the licensee to' estimate the radiation; exposure doses of individuals who had lost or missing film badges had not been documented.

This is a^ Severity Level V Violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201/ Atomics International is hereby 3 required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: . (1) the

. corrective steps which have been taken and the results' achieved;-(2) corrective, steps which will bataken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
