SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT

NO. 50 295/97032(DRP);50 304/97032(DRP)

Dear Mr. Kingsley:

On February 2,1998, the NRC completed an inspection at your Zion 1 and 2 reactor facilities.

The enclosed report presents the results of that inspection.

During this inspection period, your staff's performance continued to be characterized as

inconsistent despite a reduction in plan * , ;tivities. Specifically, control room operators failed to

adequately monitor plant parameters, w .ich contributed to the inadvertent filling of the main

steam lines with water, and operations department staff failed to use the correct procedure when

clearing an out of service for the auxiliary feedwater system. In addition, the deficient material

condition of plant equipment, specifically, leaking isolation valves, continued to challenge

operators in maintaining control of water inventory and contributed to filling the main steam lines

with water. Concerning the auxiliary feedwater valves being out of the required positicn, a non.

licensed operator demonstrated a good questioning attitude in identifying the valve configuration

problem.

In addition, we are concerned about one violation of NRC requirements that was identified. The

violation pertains to the failure to use the appropriate system operating instruction for specifying

the return to service position oi valves in the auxiliary feedwater system.

This violation is clied in the enclosed Notice of Violation (Notice), and the circumstances

surrounding the violation are dost.ribed in detailin the enclosed report. Please note that you are

required to respond to this letter and should follow tb instructions specified in the enclosed

Notice when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory

requirements.
