NRC INTEGRATED INSPECTION REPORT 50-352/97-10,50-353/97-10

AND NOTICE OF VIOLATION

Dear Mr. Smith:

This refers to the inspection conducted on November 18,1997, through January 19,

1998, at your Limerick Units 1 and 2 reactor facilities. The enclosed report presents the

results of this inspection.

During the nine-wee nspection period covered by this report, the conduct of activities at

Limerick Units 1 and 2 was generally characterized by safe and conservative operations

and decision making. Your staffs' response to an emergency diesel overpower event and a

,

failure in the redundant reactivity control system were excellent. The equipment was

returned to service promptly following thorough evaluations and corrective actions.

Additionally, the identif::stion of a mis-wired primary containment isolation valve (PCIV)

and an inadequate test regime for numerous PCIVs indicai:=s a healthy questioning attitude

within your organization.

However, the failure to properly assess operability of a high pressure coolant injection

exhaust valve in light of a number of recent failures of that valve are of concern. In

particular your engineering staff and plant operations review committee'r

ie to address

the technical specification required closing time indicates weaknesses in your process for

addressing operability and if uncorrected could lead to future violations. Additionally the

large number of wiring discrepancies with the scram solenoid pilot valve indicates

longstanding maintenance practices that are less than adequate.

The Radiation Protection program was found to be generally effective, however, minor

weaknesses were found in various program areas Minor weaknesses were found in the

accuracy of radiation protection instrument calibrat:ons and the detection capability of

whole body count measurements. The continuing training program for RP technicians has d.

been weak in providing periodic RP fundamentals revitw, which is being addressed. RP

program audits and assessments were generally effective. TLD vendor oversight was very

effective. Station material condition and posting practices were found to be excellent.

Based on the results of this inspection, the NRC has det!= ained that violations of NRC

requirements occurred. These violations are cited in the enclosed Notice of Violation

(Not!ce) and the circumstances surrounding them are described in detail in the subject

inspection report. The first violation is of concern because it involves inaccuracies in the

control room log which is used to establish numerous NRC inspection activities. The

second and third violations are of concern because they both impact the configuration

control of plant systems. You are required to respond to this letter and should follow the

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary

to ensure compliance with regulatory requirements,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosure (s), and your response will be placed in the NRC Public Document Room

(PDR).
