Enclosed is a copy of revised NRC Form 398 " Personal Qualification Statement Licensee,"
(10/97) Enclosure 1, which has been revised to reflect current operator licensing policy. The changes to the form are described in Enclosure 2.
By copy of this letter, your facility training manager is being provided with a small supply of the form and is asked to ensure that license applications are submitted on the revised forms as soon as possible but no later than May 1,1998.
