NOTICE OF VIOLATION Comonwealth Edison Company Docket No. 50-454 As a result of the inspection conducted on March 17-21, 24-27, 31, and April 1-3, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violations were identified:

10 CFR 50, Appendix B, Criterion XI, as implemented by the Comonwealth Edison Quality Assurance Program, requires that test results be documented and evaluated to assure that test requirements have been satisfied.
Contrary to the above, results of surveillance test BOS DC-21, conducted on December 10, 1985, listed two readings that exceeded acceptance criteria as specified in the procedure. Test results were indicated as satisfactory, and there were no indications that the problem was noted on subsequent reviews of the test results (454/86012-03).
This is a Severity Level IV violation (Supplement I).

10 CFR 50, Appendix B, Criterion XII, as implemented by the Comonwealth Edison Quality Assurance Program, requires that measures be established to assure instruments and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy witt.in necessary limits.
Contrary to the above, the station " Instrumentation Schcduling File Listing," issued February 28, 1986, contains numerous listings of plant instrumentation overdue for calibration. Of these overdue isstruments, at least 27 were determined to be safety-related (454/86012-04).
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
