NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on November 11, 1985 through March 6, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
10 CFR 50, Appendix B, Criterion II as implemented by the Illinois Power Quality Assurance Manual, Chapter 2, states, in part, that the quality assurance program shall provide control over activities affecting the quality of the identified structures, systems, and components, to an extent consistent with their importance to safety. Activities affecting quality shall be accomplished under suitably controlled conditions. Controlled conditions include the use of appropriate equipment;... and assurance that all prerequisites for the given activity have been satisfied.
10 CFR 50, Appendix B, Criterion XVII, as implemented by the Illinois Power Quality Assurance Manual Chapter 17, states, in part, that sufficient records shall be maintained to furnish evidence of activities affecting quality.
Contrary to the above, the following examples of inadequate records and control over activities were identified:
a.
On January 18, 1986 the applicant identified a hole in the containment liner (the leaktight barrier) that had existed for a period of approximately two years. The applicant failed to control activities affecting quality as they were unable to identify any work or inspection activity that could have created or discovered the hole.
b.
As part of the prerequisites required prior to the performance of the Type A test, the applicant performed a pretest containment inspection.
This containment inspection should have uncovered any evidence of structural deterioration which may have affected the leak tightness of the containment structure. The applicant's prerequisite containment inspection was inadequate because it failured to identify a hole in the containment liner which could have caused the containment to leak in order of magnitude greater than the maximum allowable leak rate.
This is a Severity Level IV violation (Supplement II).
I Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including for each violation:
(1)correctiveaction j
taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
