Inspection Report No. 50-309/86-08

This transmits the May 11 - June 16, 1986 routine inspection findings of Messrs.

C. Holden, J. Robertson and other region-based inspectors, of the Maine Yankee

Atomic Power Plant, Wiscasset, Maine.

Those findings were based on observations

of activities, interviews, and document reviews, and have been discussed with

Mr. Garrity of your staff.

Within the scope of this inspection, no violations were observed.

Even though your

post trip review process did not find any conclusive evidence of the cause of the

failure of the Excess Flow Check Valve rupture disk, we observed the review process

to be thorough.

Although not safety related, we found the pressure switch in the raw water to the

"B" service water pump bearing to be isolated (see Section 4.b of enclosed report).

Also, we understand that your staff has identified another isolated non-safety re-

lated pressure switch since the end of this inspection report period.

Isolation

of safety instrumentation that was a part of the reactor protection system (trip

on high steam generator pressure) was the subject of Violation 85-27-01 and subse-

quent escalated enforcement action.

I am concerned that these 2 recent instances

of isolated instruments may be indications of weaknesses in your corrective actions

for Violation 85-27-01.

Please address measures that are being taken to address

these isolation problems at Maine Yankee in your response to this lettor.

The re-

sponse directed by this letter is not subject to the clearance procedures of the

Office of Management and Budget as required by the Paperwork Reduction Act of 1980,

PL 96-511.
