SUBJECT:
NRC INTEGRATED INSPECTION REPORT NOS. 50-424/97-10 AND
50-425/97 10 AND NOTICE OF VIOLATION
Dear Mr. McCoy:
On November 1. 1997, the NRC completed an inspection at your Vogtle Units 1
and 2 reactor facilities.
The enclosed report presents the results of that
inspection.
During the six-week period, your conduct of activities at the Vogtle facility
was generally characterized by safety-conscious operations, sound engineering
and maintenance practices, and appropriate radiological work controls.
However, two violations were identified.
The first violation dealt with
alignment of Unit 1 diesel generator unit heater 480 volt AC breakers. The
second violation dealt with the APEX users manual that was to be used by
reactor engineers for restart of a unit after a trip.
This violation was
previously documented during the Unit 2 startup from the fifth refueling
outage.
However, the corrective actions developed then were not fully
implemented by the subsecuent startup of Unit 1 from the seventh refueling
outage.
We are concernec with this violation due to the failure of your
organization to complete the corrective actions.
In addition, we are
concerned with the identification cf an issue that an operations crew failed
to recognize that a Limiting Condition for Operation was entered and exited
for an emergency core cooling system.
This issue was identified in this
report as a weakness, however, we request that in your res)onse to the
violations that you also include your actions to address t11s issue.
The violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violations are described in detail in the
enclosed report.
Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response. in part. to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
