On March 18,1993 a fuel assembly shipping container with 2 fuel assemblies was being unpacked pungprocedure EN 31026 Rev
- 0. Training in the use of this procedure had been conducted with all personnel involved. Following EN 31026, the acct.lerometeis were initially verified as intact, t.ater, during the unloading operation when the lock tubes were being set up, the machanics shook the shipping container and tripped one of the accelerometers.EN 31026 paragraph 4.3.17 states that if an accelerometer is found tripped, the following actions shall be 14en; 1) notify the Wt Ainghouse representative,2) perform an irepection per Westinghouse procedure F-4, and 3) to write an Printed 2/39e 10-18.43 AM Pope 1 of 2

No PIR or Corrective AC,on Pequest was written.In order to respond to the issues identiflod in DRMP3-0193, additional clattfloation is provided below, of the actions taken by NU during this event. Training was conducted on EN 31028 and the procedure was followed.
Automated Wort 6 Order (AWO) MS 93-04694 identiflod the deflaiency, an NCR was wrteten and the inspection was performed. These actions are in socordance wMh NEO 3.05 rm NCR's. The quotation used in this DR from NEO 3.05 is correct, but it is not complete. NEO 3.05 continues to state that in the fleid, an NCR is not used to identify deflaiencies but to provide en0ineering direction. NEO 3.06 eleo st:tes that deflaiencies are identified by several possible vehwies including an AWO, which was done. NEO 2.18 for corrective actions states that a CAR is used when no procedure exists to correct the problem in this case a procedure did exist as the EN directed the field to inspect the fuel assemblies using Attachment 3 to Westinghouse xocedure F-4 and write an NCR. The threshold for wetting a PIR n this case was dependent on the oute:,me of the F 4 inspection.
No damage was found and therefore no PIR was required. in summary, procedures existed to cover the adions taken and they were appropriately followed. Significance Level crtteria do not soply here as this is not a discrepent condMion.
NU has conclMed that the issue reported in Discrepancy Report, DRMP3 0193, does not represent a discrepent condRion.
Procedures were in place, training was provided and appropriate actions taken in response to the event. 86 nificanos Level.

NU has concluded that Discrepancy Report, DR MP3-0214, has identified a condition not previously discovered by NU which requires correction. This discrepancy report identifies a missing jumper between terminals TB14 C65 & C65 (common 125VDC power) in 3lHA ANNMB2 on Test Loop Diagram (TLD) 3QSS-060. This drawing deficiency was reviewed and found to be valid. The annunciator logic cabinet drawing EE-4R reflects this jumper as a common 125VDC power connection on the manufacturer's side of the terminal block. TLD 3QSS-060 has to be revised to add a jumper between terminals TB14 C65 & C65 in 31HA-ANNM82 to agree with drawing EE-4R. Conoition i

Northeast Utilities ICAVP DR No. DR4PW33 Millstone Unit 3 Discrepancy Report The control adion depicted as PB 3SWP*MOV102A Open/ Auto (rimilar for P8-3SWP*MOV102C) on LSK 910K and PB-SSW*MOV1028 Open/ Auto (similar for PB 3SWP*MOV1020) on LSK 9-100 indicates these valves can be opened manually by the pushbutton or automatically by their reapedive SWP pump running. The Remote / Isolate switch at the Fire Transfer Switch Panel Inust be in the REMOTE position for 3SV"MOV102A & 3SWP*MOV102C for this to occur. Since there is only one adlon on this pushbutton as shown on ESK 3K, detail BQ9, it would be incorred to represent this action in two places on the logic diagrams as suggested by this DR. Also, PalD EM 133A correctly details the func/lonal information for all four valves mentioned. Therefore no change is required by this item.

- 1. Gate valve desyn spedfication 2282.050476 through Revision 1 shows valves 3RSS*MOV38A.B not to have stem leakoff connections. However, drawing 2282.050-676110 Revision E shows the valves to have lantem ring stem leakoff connections. Pecking within the stuf'ing box is not addressed in specification 2281.050-676 or drawing 2282.050-676110.
- 2. Valvo design specification 2282.050-676 through Revision 1 shows gate valves 3RSS*MOV8837A,B and 3RSS*MOV8838A,B not to have lantem ring leakoff connections. However drawing 2282.050-676103 Revision i shows valves 3RSS*MOV8837A,8 and 3RSS*MOV8838A,B to have stem leakoff connections. Adverse Condition Report (ACR) MS 96-0357 documents an inadequate description in Licensing Event Report (LER) 96-005-000. The original concem, iden;ified by Vertical Slice Review Team item #506, was written to address a design noncompliance created by Bypass Jumper 3 90 20. This ACR was incorredly categortzed as level a *D." The ACR documents that the LER did not report the design basis violation associated with the bypass jumper. Such an omission from an LER warrants a significance les el higher than level *D." This document should be categorized at the same level as the o'iginal problem. A significance classification higher than level'D' would have resulted in a re-review of the entire investigation. This investiga'lon would address the lack of sufficient discussion regarding violation of the plant's design basis documents (DBD) and assessed the completeness of the LER's revision. According to NUREG 1022, errors in an LER must be corrected in a revised report, and this revised report must be a stand alene document. Since the original dispocition determined that the condition crea'ed by Bypass Jumper 3 90 20 was reportet,le, and the resul? Of an inadequate safety evaluation, the subsequent disposition revising the LER should have re-reviewed the original Root Cause Anal, sis and other supplementalinvestigations to determine the adequacy of this revision. Additionally, the ACR's Corrective Action Plan stated that there is additional discussion being added to the causal f actors determination and corrective action; however, this revision is inconsistent with RP 4's requirements for dispositioning level'D' ACRs. Level"D' ACRs do not require a causal factor deteimination.

The operating pressures and temperatures are determined la Cciculation P(R) 1187 Rev. 2.
Four discrepancies were identified in Calculation SDP RSS-01361M3, Rcv. 5:
- 1. (p.18) The design pressures and temperatures do not reflect those computed in P(R) 1186, Rev. 2. Design pressurn at pump suction should be 49 psig, design pressure at pump discharge should be 300 psig, design pressure at containment isolation valves MOV*20NB/C/D should be 283 psig, design pressure at Si crosstle valves MOV'6837NB & 8838NB should be 285 psig, and design temperature should be 260F throughout the entire spray circuit.
- 2. (pp.19 21) There is no source for the 39 psig operating pressure for suction piping in Operating Condition 1. This parameter is not computed in P(R)-1187, Rev. 2.
- 3. (pp. 20 21) The operating pressures !n pump suction piping for the Operating Condition 3,3RSS*P1 A test mode, agree with values computed in P(R) 1187, Rev. 2. but contradict the 50 psig value computed in US(B) 1186, Rev,2,
- 4. (pp. 20,21,25, and 26) The operating pressures in pump dewatering lines 3-RSS 150-43,51,84, and 87 are identified as equal to it.ose for the pump suction piping even though these lines are located nearly 17' 2* below the suction piping (an elevation difference which corresponds to as much as 7.4 psid).

No documentation was provided that could be used to verify OPS Forms 3306-1 & -2, VALVE LINE UP FOR CONTAINMENT RECIRCULATION SPRAY SYSTEM (TRAIN A & B), were revised to reflect the new flanges.
OPS Forms 36128.4-80, -81, -82, & -83 were revised to Indicate that 'AWO to MNTC-blank flange to be installed in the CTMT sump". No documentation was provided that could be used to vrAfy an AWO to maintenance or some other method of controlling the removal of the blind flange after the local leak rate test (C.LRT) was generated.

The LLRT test procedure SP36128.4 addressed removal of equipment installed for testing.
