Inspection Nos 30-04545/83-01; and 30-06931/83-01 This refers to the routine safety inspection conducted by C. Rowe of this office on April 13, 1983, of activities authorized by NRC License Nos.19-08330-02 and 19-08330-03 and to the discussions of our findings held by C. Rowe with you and members of your staff at the conclusion of the inspection, and to a subsequent telephone discussion between C. Rowe and Dr. N. Chawla on April 20, 1983.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and the conditions of your licenses. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, measurements made by the inspector, and observations by the inspector.
In addition, our inspector took water samples from your cobalt-60 irradiator-pool and waste holding tank which were analyzed in our Regional Office Labora-tory in King of Prussia, Pennsylvania. The basic purpose of these independent measurements is to verify your capability for analyzing concentrations of radioactive material in water.

categorized by severity level in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register Notice (47 FR 9987) dated l
March 9, 1982.
You are required to respond to this letter; and in preparing your response, you should follow the instructions in Appendix A.
i From the telephone discussion between Dr. Chawla of your staff and C. Rowe of this office, it is our understanding that all' transfers of, licensed radioactive material between the Naval Medical Command and AFFRI will be made in compliance with the Department of Transportation regulations.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2 Title 10, Code of Federal Regulations, a copy of this letter and your reply will be placed in the Public Document Room.

APPENDIX A NOTICE OF VIOLATION Defense Nuclear Agency Docket No. 30-04545 Bethesda, Maryland 20014 License No. 19-08330-02 As a result of the inspection conducted on April 13, 1983, and in accordance i
with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:
Condition 19 of License No. 19-08330-02 requires that licensed material i
be possessed and used in accordance with statements, representations and i
procedures contained in a letter dated February 2, 1981.
This letter incorporates the Radiological Safety Instruction for the Armed Forces l
Radiobiology Research Institute.
[
Radiological Safety Instruction 140 incorporates HPP-03, " Source Receipt and Notification." Section 6 of HPP-03 requires that an immediate report be made to a member of the professional staff if a package is received with a contact radiation level in excess of 200 millirem per hour.
Contrary to the above, a package containing a molybdenum-99 generator received on April 13, 1983, was observed to have a contact radiation level reading in excess of 200 millirem per hour and the required notification was not made.

Pursuant to the provisions of 10 CFR 2.201, the Defense Nuclear Agency is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this respor.se time.
