This letter confirms the comitments made by you at the NRC Region III office during an enforcement conference held on May 12, 1986. These commitments were further discussed on June 17, 1986, between Mr. D. L. Farrar of your staff and Mr. C. W. Hen 1 of this office. The enforcement conference and subsequent conversation related to our concerns over the event in which a Nuclear Station Operator (NS0) entered the initials of an Equipment Attendant (EA) on a check-list and certified that a second verification of the position of a motor-operated valve breaker had been perfor.med. As a result of the NS0's act, the second verification was not performed.' The event became more significant when the NSO denied his action on two occasions when questioned by station personnel and did not acknowledge his actions until 12 days later.

With regard to the matters discussed, we understand that you will complete the following actions:

1.

You will conduct an Audit Program whereby Comonwealth Edison Company technical staff and management personnel who are not assigned to the LaSalle site will monitor the activities of the NS0 to determine that the NSO would implement his assigned duties with integrity and a sense of responsibility while not being audited. This program will be submitted to the Regional Administrator for review and approval.

2.

The audit will continue until such time as the Regional Administrator is satisfied with the determination that the NS0 will, in the future, implement all duties with a sense of integrity and responsibility.

3.

Prior to termination of the Audit Program, you will provide the basis for your conclusion regarding the integrity and responsibility of the NS0 to the Regional Administrator.
