NRC INSPECTION REPORT NO. 50-456/97016(DRP); 50-457/97016(DRP)
AND
Dear Mr. Tulon:
On November 3,1997, the NRC completed an inspection at your Braidwood Units 1 and 2
reactor facilities. The enclosed report presents the results of that inspection.
During the 6-week period covered by this inspection, good performance was specifically
noted duririg the Unit 2 shutdown for refueling, the observations of surveillance testing, and
the performance of some maintenance activities. We are concemed, however, about the
failure to take effective corrective actions in several areas and a general station wide failure
to follow procedures.
Six violations were cited in this report. Two violations were cited for the failure to take
effective corrective actions to address previously cited violations. The inspectors identified
seventeen examples of failing to adhere to the procedures for the control of carts and
moving equipment. The failure to control carts and moving equipment had been previously
discussed in inspection Report No. 96021. The inspectors identified ten examples of failing
to follow procedures for the control of combustible materials. Tlie failure to follow
combustible material control procedures had been discussed in Inspection Report No.
97007, in addition, licensee perconnel identified eleven mispositioned components between
September 2 and October 4,1997. Problems with mispositioned components have been
discussed in previous inspection reports and this is a recurring issue. A violation .~ar the
failure to take ef'ective corrective actions for the incorrect positioning of equipment was
cited in Inspection Report No. 97015 and the examples identified during this inspection
period are considered to be additional examples of that violation.
Three additional cited violations and one non-cited violation involved the failure to follow
procedures. During August and September 1997, the inspectors identified five examples of
failing to follow the scaffold construction procedure, while ; tensee personnel identified
another seventeen examples. The inspectors identified that contractor personnel were not
following the work package instructions on how to initiate a freeze seal prior to the
beginning of maintenance. The inspectors also identified a radiation area within the plant
that was not posted in accordance with the procedure. Licensee personnel identified that a
procedure was not followed when a modification was made to the main turbine run back

termination setpoint on both units in 1990 and control room operators were never trained on
the change. Since this issue was identified and corrected by your staff, it was considered a
non-cited violation.
