- 1) In the trip circuit of the Service Water Pump (3SWP*P1B) breaker, the schenutic diagram (ESK 5CK Rev.16) indicates a connection between terminal point BB 12 and point 67 on contact 67 68 of handswitch 52S 3SWP*P1B. T'1e corresponding wiring diagram 25212-39010 sht. 498 Rev. F, at terminal point BB 12, does not indicate this connection.
- 2) Schematic diagr%m (ESK SCN Rev.13) for 3RSS*P1 A indicates an electrical connection between terminal point AA 3 and handswitch 43-3RSS*P1 A (contact 5), in the closing circuit of the breaker. The corresponding wiring diagram (25212 39010 sht. 449 Rev. F) does not indicate a connection between these 4
two points.
- 3) The closing circuit of the RSS pump breaker (3RSS*PI A -
schematic diagram ESK 5CN Rev.13) Indicates a series

- using handswitch 43-3RSS*P1 A (contset 10) and auxiliary relay 1X-3RSS*P1 A (contact 17). This portion of the schematic diagram doec not match the corresponding wiring diagram 25212 39010 sht. 449 Rev. F. As an example, this wiring diagram Indicates that both sides of contact 10 (43 3RSS*P1 A) are both connected to point 7 of auxiliary relay 1X-3RSS*Pi A.
- 4) Schematic diagram (ESK-5DH Rev.11) C 3S*P3B indicates a connection between handswitch (n.,QSS*P38) contact point 3C and handswitch (43-3QSS*P38) contact 13C.
The switchgear intemal wiring diagram 39010.sht. 484 Rev. E does not Indicate a connection between these two points. In addition, schematic diagram ESK-5DH Rev.11 Indicates a seal-in contact (12) on the reset coil of relay 86E. The switchgear inieman wiring diagram 39010 sht. 484 does not indicate a seal-in contact.
- 5) Schematic disgram ESK-6ALG Rev. 7 (3SWP*P3A &.
3SWP'P38) indicates the use of a General Electric CR120BL relay, which contains an " operate & reset
- coil. The corresponding wiring diagrams refers to wiring diagram EE-9JS Rev. 2, figure 11H, for the wiring of these relays (3 3SWP*P3A

D*ecription: This calculation was prepared to establish the maximum permitted cable lengths for use in iaotor control circuits. The calculation determines the maximum permitted cable lengths based only upon voltage drop.
Small control relays should also be reviewed because of their small holding current requirements to determine whether the capacfilve current contained in lengthy, multi-conductor cables may be a problem by preventing relay dropout after the circuit is interruptad. Mutual capacitance should be addressed in the calculation to determine the maximum allowed cable lengths.
Per CCN 2 of the calculation, the maximum allowable cable length for the Size 1 starterwhich has GE CR120B relays rated at 15 Holding VA is 5814 feet fur a #10 AWG cable.
For this application, S&L's standard would allow a maximum cable length (cable length being equal to 1/2 of the circuit length) of approximately 2400 feet (41% of the length allowed by NU).

Plant Design Change Request (PDCR) # MP3-92-013 replaced four time delay relays in the Service Water Pump Strainers. The PDCR was identified as a AQ, Cat. I activity and that the new time delay relays were required to be qualified as class 1E devices. Further, the PDCR documented the requirement to up date procedure PT 31459A, MP3 Timing Device Calibration s
Program. Revision 2 CH 2 of this procedure which has an effective date of October 1,1997 does not include the replaced time delay relays.
NU's Quality Assurance Program (NUQAP) states that ' Periodic c9libration and adjustment of measuring rN test equipment is performed and controlled to assure accuracy is maintained within limits necesury to verify and control to assure accuracy is maintained within limits necessary to verify that design and s
operating condition requirements have been met. The ope ating requirements for these relays is identified in the PDCR as providing a safe and reliable means of allowing the system to remain in its automatic mode to provide automatic gross filtering of the service water system cooling water.
No documentation was provided that would verify the service water pump strainer timers were included in the Mill *one Unit 3 timini device calibration program or weie calibrated on a scheduled frequency.

Levelindicators 3EGF Ll29A1,3EGF LI-29A2,3EGF-Ll2981, and 3EGF-Ll2982 do not meet this requirement, The scales presently installed on the referenced indicators show a range of 0 - 530 gals, they are hand written not machine printed, and have red printing on white scale plates, not the required black printing on white scales.

instructions forlabeling of abandon equipment, instruments 3HVR*FS27A,3HVR*PDT208A, and 3HVR*PDT208B have been abandoned in place and they do not have the required labelir g.
- 6. The following differences between the installed components that the requirement of the referenced design documents were noted during system walkdowns for field installed instruments:
3HVR TIS 109A,1098,109C, and 110. Vendor drawing 1179-2472.900-609-249, Rev. B, Indicates that an expanded metal (16 gauge) protective screen is to be mounted over the sensing bulb and that the sensing bulb is to be mounted using a % inch conduit clamp, with insulating tape sleeve, and in an upward direction. The protective screen could not be found and the instrument is not as described on the vendor drawin0s.

- and improperly cicsed Deecription: UIR 776 is incorrectly identified as not start-up requirtsd. This UIR states dat pump 3QSS*P3A was run dead headed for about 10 minutes (with indications of some increase in temperature, and that the pump had not been evaluated for potential damage.
The corrective action for the UIR was the init '3n of an AR for the system engineer to determine if additional action /surved!ance is required for the pump. The UIR was closed based on this AR, Closure of the UlR based on the identified corrective action is inadequate due to potential operability problems. The " potential damage evaluation" should be required PRIOR to startup.
Additionally, an ACR should have been initiated, tather than ar.
AR, as was done in a similar situation for pump 3Q.cS*P1 A tinder UiR 744.

Conduits to this raceway from Instruments 3HVR-PDIS918,938, and 94 B,157A,157B do not shcw the wireway as a from/'o for the conduits; but shows the from/to as "end".
- 9. Conduit 3CC11500 did not have an identification tag installed.
10.-Junction boxes 3HVR*JB1-1 A and 3HVR*JB1-2A have erroneous instrument information listed on the box in the field.
The boxes are also mis-identified on drawing EB-45G Rev. 9.
Field and drawing indicate boxes are for 3HVR*TS175A-1/ A-2 and 3HVR*TS175A 3/ A.4 respectively. The correct designations for these boxes are 3HVR*TS175A 1 and A-2 respectively. Similar situation exists for instruments 3HVR*175B-1 and B-2.

- 3. Conduit 3CX449NX3 is listed on Conduit Support log 12179-FSK DG-129, Rev. 4A, as attached to the support. This conduit is not attached and as a result its span of greater than 7 feet 4 inches exceeds the allowable.
- 4. Conduit Support Log 12179-FSK CB 555, Rev. 8, in Section 1 shows all conduits attached to the top of the horizontal members. This is not accarate; all conduits attached to de B member are below as is 2 of the 4 on the C member.

- 1. On page#12, Angle 4x4x3/4 is analyzed and due to interference, Angle 4x3x1/2 is used in the field.
This is noted in the calculatioli and accep4d by engineering judgernent. Angle 4x3x1/2 has lower sectional properties and it is not structurally adequate as shown below.
Ratio of A,eas= 5.44/3.25=1.67 sq. In.
Therefore, actual axial stress =fa=11704x1.67=19590 psi KUr (min)=1.0x64/.639=100 Therefore, allowable axial stress =Fa=12.98 ksl < fa=19.59 ksi (NO GOOD)
- 2. Angle sections are not checked for r.ombined axial and bending stresses and also the AISC CODE check is not performed in STRUDL RUN.
- 3. On page#13, for Angle 6x4x1/2 axial stress ratio fa/Fa=.Pi>,15 and it is quahfied by engineering judgment.This is unacceptable methodology.

Date Putd6ehed: 1/259e Descrepency: No program has been established to measure DG cylinder firing pressures and evaluate results Deecripuon: The letter from Northeast Utllities to the NRC dated June 14, 1995 (B15245) statec that '...we have decided to implement a supplemental Inspection in order to better evaluate engine performance. Cunently only rock settings and temperatures are monitored during diesel surveillance runs. Maintenance on a six-month basis will measure cylinder firing pressure and evaluate the results. This will allow an evalustion of the engine timing (retard or advance) and fuel mixture (too rich or too lean).*
RFI 0824 was submitted to obtain the procedure and data sheet that is used to perform the task as described in letter 815245.
The RFI provided a copy of Maint Form 3720CD 1, Rev 3 and referred the reviewer to MP 3720CD, Rev 4 even though OSCAR identifies a Rev 6. The RFI also requested copies of the three most recent uses of Maint. Form 3720CD 1 which had been provided earlier in response to RFI 0715.
Contrary to the above comm!! ment, the reviewer was not able to confirm the following:
- 1. A program has been established to perform a twice year 1y evaluation of the diesel cylinder firing pressures.
- 2. That when data is collected on Maint. Form 3720CD 1 that there is a documented process to calculate the hi0h/ low differential exhaust temperature and firing prer ',ure.
- 3. That any documented evaluation is performed to determine proper engine timing or fuel mixture.
- 4. A process that describes follow-up actions that the evaluation might require.
The work orders associated with " copies of the three most recent ucas of Maint. Form 3720CD-1' provided were dated 9/6/93, 2/1/94 and 5/16/95, it therefore appears that this surveillance has not been performed since 5/16/95 which includes all of 1996 and 1997.
The RFI also requested copies of evaluations performed as a result of the testing program. The site responded that "there have been no evaluations performed because cylinder pressures have not exceeded the limits and rework was not performed."
The subject letter appears te commit the station to a twice yeariy cylinder firing pressure surveillance test of the diesel which includes a subsequent evaluation for engine timing and fuel mixture, it cannot be determined that this program is in place and therefore this is a discrepancy.

As part of that review the following discrepancies were noted.
- 1) The E 3 procedure cites a 46-minute time for completion of the adions necessary to complete plant cooldown to cease the tube rupture break flow and terminate the release. it is cited in the purpose of the procedure and Indicated as consistent with the FSAR. The ICAVP team notes that the FSAR cites a 30-minute time period for this action and it,s associated offsite release assessment uses the 30-minute time for the rolesse duration.
- 2) The FSAR Indicates that the operator controls the ruptured steam generator at the steam generator relief valve setting. The E 3 procedure diiects the operator to control the ruptured steam generator at a pressure of 60 psi below the relief valve setting.
This inconsistency could cause an under prediction of the release from the RCS to the steam generator.
NU has identified in UlR 1061 that the affected safety analyses for the steam generator tube rupture need to be updated.
However, the assumptions and EOP inconsistencies identified above should be reviewed and factored into the safety analyses and, if determined to be appmpriate, coordinated with EOP revisions.

The ICAVP has reviewed the reference EOP as part of the verification of plant charaderistics important to ensure that safety analysis assumptions are appropriately implemented and maintained in accordance with the MP3 FSAR Chapter 15 accident analysis. As pa t of this verification the ICAVP reviewed the instrumentation necessary to ensure that the cooldown necessary to reduce the RCS pressure and minimize the release is initiated and appropriate for the actions ri described in the EOPs. It is noted that the entry criteaa for the E-3 EOP is based on the operator's review of the radiological conditions using the steam line radiation, condenser air ejector radiation or steam generator blowdown radistion monitors.
Given the importance of this diagnosis to the initiation of the cooldown actions necessary to mitigate a steam generator tube rupture in accordance with the applicable safety analysis for the plant, the ICAVP would have expected these monitors to be Reg.
Guide 1.97 Type A variables. A review of the MP3 Reg. Guide 1.97 compilance analysis and FSAR indicates that they are designated for monitoring only as Type E variables. This issue is of importance due the origirial compliance witt Reg. Guide 1.97 and the recent emphasis of diagnosis and response to steam generator tube rupture events to prevent overfill.
NU should address which, if any, of the identified Reg. Guide 1.97 Type A, Category 1 instruments has been designed to monitor this condition.

item 1 DCN DM3 S-1506-92 (page 6) Indk.ated that cable 3HVRCOC700 was to be wrapped with Sil Temp to resolve an apparent separation vlo!atlon. Field observation revealed that this cable was not wrapped, however, adjacent conduit floor sleeve was extended and no separation vlotation was observed.
No change documentation could be found to eliminate the requirement for wrapping of the cable.
Item 2 DCN DM3 S 1495 92 required the removal of wirirc, from TB-4, points 1 and 2 at Panel 3CES*1PNL120. The DCN was incorporated into revision 4 of panel drawing EE 3NV. The current revision of the drawing (Rev. 3) dot.s not show wires connected to these points and none of the outstanding change documents for the drawings add wires to these points. Contrary to this, a black wire is landed on point 1 and a white wire is landed on point 2.
Item 3 Changes required by this DCN DM3 S 1606-92 included: the removal of an intemal jumper from terminal block 3T71 and re-landed at point 7; and the relocation of an intemal jumper between 3T7 points 8 c.nd 9 to points 1 and 7. This DCN has been incorporated into the affected drawing (ES3PH) with the curTent revision (Rev.10) showing these changes. Field
-inspection found these changes not installed with the current field wiring as depicted in the 'before* diagram within the DCN.

- 1) On page 6, data item #1; a sump water temperature of 215'F based on reference 5 (calc US(B) 356, Rev.1) is used. This is in conflict with the 260'F sump water terr.perature stated in Safety Evaluation S3-EV 97-0045 prepared for DCR M3 96054,
- 2) On page 6 & 7, data item #2; uses a 230*F water temperature downstream of the safety injection pumps. This does not agree with page 92 of reference 5 that states that the temperature downstream of the safety injection pump is 240 'F.
- 3) On page 7, assumption #2; assumes that piping temperature will approach 230'F. This does not agree with the 260*F stated in 3afety Evaluation S3-EV 97-0045 item 1.1
- 4) Page 8, piping heat losses need to be recalculated using the higher pipe temperatures noted in Safety Evaluation S3-EV 0045.
- 5) Page 8, calc for 10" line with 1.5" of encapsulated insulation and 8" line with 2" of encapsulated insulation; states room temperature is 120*F but uses room temperature of 110'F when adjusting heat loss.
- 6) Page g, item 7 & Table 5; Heat gain of 14,430 Btu /hr for ACUS 2A/B fan heat gain (page 65 of original calc) needs to be added to totals shown in Table 5 for rooms 1 A & 1B Calculation Change Number 4:
- 1) On page 5 of CCN, item 2; the CCP leaving RHS heat exchanger water temperature of 142*F does not agree with reference 3 (SGCS Calc #95-052 Rev A). With a 115 'F CCP temperature entering the RHS heat exchanger, the leaving tcmperature is approximately 150 *F. The 142*F temperature used in the calculation appears to be based on a 10g - 110 'F CCP temperature.
- 2) On page 6 n' CCN, a two foot floor thickness was used for area 4E. Drawing EC-328 7 shows the floor thickness as three feet. Using the three foot thicknes increases the R value and reduces th; heat transfer thru the floor.

- 4) Calculation assumes a grcand temperature of 50 'F. Based on review of average earth temperatures on page 11,4 of the 1991 ASHRAE Applications handbook, a ground temperature around 70'F would be a more appropriate assumption. This reduces the available temperature difference from 54 to 34 'F for a 1047 room design temperaturn,
- 5) Page 5 of CCN refers to a Reference 5. There is not a reference 5 identified in the reference section of the CCN.
- 6) in Table 3 for the 1rt 24" line in room 2A and 28, the linear foot factor from Table 2 should be 6.283 instead of the 6.383 used.

Plant Design Data System (PDDSS data for service water pump 3SWP*P1 A and 3SWP*P1B states that the motor rating is 600 hp v1th a brake HP requirement of 5.8.5 hp.Two pumps operate per. LOCA per FSAR section 9.2.1.2 while the calculation only considered one operating. This would resu't in higher than calculated heat loss to the pump room imm the service water pump motors.
Calculation P(B)-901 Indicates that P(B)-906 supersedes calculation P-198, However, calculation P-198 is still sh7sn as an active calculation in the calculation database and P(B)-906 has been superseded by P(B) 925.
- 2) Calculation P(B)-901 considered only one pump operating with a bhp of 561, Two pumps operate post-LOCA per FSAR section 9.2.1.2 which would result in higher thaa calculated heat loss to the pump room from the service water pump motors.
- 3) Calculation P(B)-920 considered only oe pump operating while two pumps operate post-LOCA per FSAR section 9.2.1.2
- 4) Calculation P(B)-925 used motor load values from calculation P(B)-901 which only considered one pump operating. Per FSAR Section 9.2.1.2, two pumps on the same division are required to operate post-LOCA.

DR MP3-0115, does not rcpresent a discrepant condition. TRM Table 3.6.3-1 has been changed, effective 8/27/97 (Revision 6) prompted by continuing revisions to the calculation. The change eliminated the stroke time column.
Note 3 cf Revision 4 indicates that the stroke time value is the surveillance limit (see attached copy of 3613F.21) and may be more limiting than other documents.- TRM-3.3.2 is a clarification of response times for Tech Spec 3.3.2 ESF actuation signals, not the Surveillance limit.
This is not a discrepancy as the 3 second requirement of 3.6.3-1 is a surveillance limit set up to intentionally be more conservative than Tech Specs, TRM-3.3.2 and the FSAR.

Specification 2170.430-065 identifies a 1500 cfm rated airflow for the HEPA filter which corresponds to a 9,000 cfm maximum airflow for the unit.

Date: 1/14/98 Resolution NU has concluded that Discrepancy Report, DR MP3-0589, has identified a condition previously discovered by NU which required correction. The closure request for UlR 1073 addresses the issue of the velocity criteria used for the testing of the subject charcoal. AITTS rid not inciude all of the information contained in the Closure Request. This information has since been included in AITTS AR 96033919-01 for reference.

Millstone Unit 3 Discrepancy Report As stated in the closure request, although the current stocks of charcoal were initially tested at a face velocity of 40 FPM, routine surveillance testing, performed at a maximum face velocity of 48 FPM for all applications, has indicated the acceptability of the existing charcoal. Because the testing is completed on a representative sample, all charcoal from that procurement lot, includirg charcoal rtored in the warehouse, is validated.
Future procurement of charcoal will require Engineering review prior to ordering due to a HOLD against the stock code number (see attached printout from Material Information Management System, MIMS). This review will be done to the requirements of Specification 2170.430-065.
NU concurs with the significance level of this previously discovered DR.

The Auxiliary Building Filter Train A Charcoal Sample Test (OPS Form 3614A.21) completed 12/5/95 used a test condition of 70%
Relative Humidity. ANSI N509-1980 Table 5-1 ' Performance Requirements and Physical Properties of (Unused) Activated Cart >on' requires a test condition of 95% Relative Humidity. This difference was not evaluated in Engineering Evaluation M3-EV-970224 Rev. O provided with NU's response.
