The following information is in response to Inspection Report No, 50-321, 50-366/86-09 dated April 22, 1986.

The report concerns the inspection conducted by Messrs.

Bearden from February 22 to March 21, 1986, VIOLATION:

Unit 2 Technical Specification (TS) 6.8.1.a requires that procedures shall be implemented for Emergency Core Cooling Systems (ECCS) in accordance with Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978.

Contrary to the above, on February 25, 1986, the Core Spray (CS) system jockey pump, 2E21-C002A, was found in a turned "0FF" position rather than in its "AUT0/RUN" position, as required by procedure, 34S0-E21-001-2, Revision 0 This pump had been turned "0FF" on February 22, 1986, for oil addition, and equipment status change was not entered into the Plant Operator's Log, as required by procedure, 31G0-0PS-007-0S, Revision 0 This is a Severity Level V violation (Supplement I).

RESPONSE TO VIOLATION:

Admission or denial of alleged violation: The violation occurred.

Reason for violation:

This event occurred because of the failure of Operations personnel to maintain the Plant Operators Log current with regard to the status change of the Core Spray Jockey Pump.

Corrective steps which have been taken and the results achieved:

Involved personnel were counseled and a Departmental Directive (OPS-86-04) was issued which stresses the importance of proper log keeping, especially in the event of an equipment status change.

Corrective ste)s which will be taken to avoid further violations:

The above actions are sufficient to prevent recurrence.

Date when full compliance was achieved:

Full compliance was achieved on March 21,1986, when the Core Spray Jockey Pump was put back in operation.
