I As reqtiesfed by your office, please find enclosed the results of our investigation into the subject nonconformance.
" Description Of Nonconformance: On the 14th of April 1986 Byron Jackson Pump Division pas notified of a concern over the ylsual appearance of valve guide rails. The valve, assembly is constructed of 300_ series Stainless Steel.
During inspection of disassembled valves, the guides appeared to be rusted.
Byron Jackson in conjunction with the own'er's designee, Combustion Engineering, r. greed to perform a chemical overcheck on the, guide rail material. This chemical overcheck revealed that the guides were carbon steel.

" Investigation into The Cause Of Nonconformance: Our investigation has concluded that on February 1979 eight 10" guide rails under part no. 77844-10 were issued from our stock. This is sufficient quantity for four valves.
During inspection, they were rejected for a reworkable dimensional nonconformance and for the part nc. not matching what was required on the Bills of Material. The dimensional nonconformance was dispositioned to rework, however, the part no. discrepancy was not addressed.
When the item was reworked, factory personnel presumed the disposition also required that the rework was to correct the part no.
As a result, the part no. was changed and the guide rails were released for assembly.
" Effects Of Nonconformance On Other Nuclear Power Stations None. This nonconformance is isolated to the subject nuclear plant. We have reviewed our Quality Assurance Records for similar discrepancies and have found no other nonconformances of this nature.
" Status Of Nonconformance: The valve guides have been removed and replaced with the correct material."
