Dear Mr. Bouletta:

This letter forwards the SALP report for Pilgrim Nuclear Power Station (PNPS) for the period April 7,1996, through September 13,1997 (Enclosure 1). This assessment was conducted using the SALP process that was implemented by the U.S. Nuclear Regulatory Commission (NRC) on July 19,1993, in this process, the NRC evaluates the performance of licensees in four functional areas: Operations, Engineering, Maintenance, and Plant i

Support. The Fiant Support area comprises radiological and effluent controls, chemistry, security, emergency prcparedness, fire protection, and housekeeping.

Performance of the Pilgrim Station was generally good, however, continued management j)'

attention is needed to address issues in several areas. The facility operated well with few h

operational transients. The material condition of the plant improved and safety related l

l equipment reliability continued to be very good. Human performance variod with some j

areas showing improvement from the previous SALP period. Performance during the last p

refueling outage that ended in April 1997 improved over the previous outage with respect J I to overall control of work activities. Improvement in the oroblem reporting process generally resulted in the identification of problems at a lower threshold. However, f "l 4

weaknesses were observed in your corrective action efforts as evidenced by persistent problems with procedure quality and use and by failures to aggressively pursue resolution of several potentially significant design issues and operator workarounds.

Performance in the operations area was rated Category 2. Improvements were neted in the control of operational activities and in the performance of initiallicensed operator candidates. Notwithstanding licensee efforts to lower problem reporting thresholds, the

NRC continues to identify degraded equipment / process issues which were missed by I

operators on rounds and management on tours. Procedure quality issues and instances of if improper use of procedures persisted. Additionally, two longstanding operator I

workarounds, that impacted scram recovery, were not aggressively resolved.

l Perfnrmance in the maintenance area was rated Category 2. Improvements were noted in I

operation and power reductions. Like operations, some improvement was noted in human performance but procedure quality issues and instances of improper use of procedures persisted.

4 Performance in the engineering area di: lined to Category 2. Engineering management oversight and involvement in routine activities and emergent plant problems remained enerally good throughout the period but some performance problems arose. Problems were noted in the interfaces among the design, installation, and the ALARA organizations associated with emergency core cooling system strainer replacement modification.

Further, significant problems were noted in maintenance of the licensing and design bases for the plant. Those problems reflected some important weaknesses in your self assessment and corrective action processes.

Perfo;mance in the area of plarit support was rated Category 2. Radiation expost;es at Pilgrim continued to be high despite efforts to address the problem. Performance in the emergency p.eparedness, security and fire protection areas continued to be good.

Housekeeping was good, but the access to some areas in the reactor building was affected by radiological conditions. A noteworthy accomplishment this period was the successful transfer of offsite emergency response reception centers.

We have scheduled a management meeting with you at 1:00 P.M. on November 13,1997.

At the meeting, you should be prepared to dircuss our assessment and the initiatives you have ongoing or plan to take to address the weaknesses detailed in our evaluation. provides the schedule of NRC inspections of your facility planned for the next year. We willinform you of any changes. The rosine resident inspection effort is not included in this schedule. We appreciate your cooperation.
