Jon November 30,1997, the NRC completed an inspection at your Millstone 1,2 & 3
reactor facilities. The enclosed report presents the results of that inspection.
!
Progress continues in the review, update, and, as appropriate, the closure of a number of.
.
issues identified in the Unit 3 significant items list (SIL). .During this inspection period,
additional programmatic SIL areas involving follow up of NUREG-0737 and Generic Letter
8913 issues were evaluated, but remain open. We note that considerable attention isi
t ;-
being directed to reconcile the Unit 3 docketed rec,ord with current commitments and
practices relative to these generic program areas. In light of past configuration concerns at
Millstone regarding compliance with docketed commitments, strong management emphasis
,
-on maintaining both accurate and technically correct compliance with program
requirements is essential to continued recovery.
~ As detailed in the enclosed inspeEtion report, the closure of the SIL item on your electrical
separation program after several months of inspection review represents a dedicated effort
on the part of your staff to identify and correct both known and suspected problem areas.
Such a broad and complete ' response to other identified areas of concern and the remaining.
programmatic SIL items will likewise require a concentrated dedication of the proper
!
' resources to bring these issues to satisfactory conclusion. In the future, your performance -
l
will continue to be measured not only in the way your staff resolves the identified open
- issues, but also by the acceptance and timely response to all new challenges, while still'

' maintaining a good operational focus on the safety of the plant during both routine
Levolutions and special tests.

. At Unit 2, in response to the NRC's request for information pursuant to 10 CFR 50.54(f),

inspection of the deferred items found that you had improved the review and approval -
process to provide additional assurance that the list would be complete and accurate. ;The-
z process resulted in the list containing items that were appropriate for deferral and overall

significant safety concern during plant operations.

At Unit-1, operations response to an unplanned entry into a Red shutdown risk condition,
as a result of a loss of the ."D" service ' water pump, was less than adequate.. During the
'
event, command and control of the operating shift was weak and without clear
expectations as to who was responsible for making the appropriate decisions to return the
>
'
unit to a Yellow shutdown risk condition. Discussions in the control room lasted for almost:
i
two hours before a decision was made on a course of action to restore defense-in-depth for
-
the ultimate heat sink. This event demonstrates the need for continued management
-
support 'and direction to ensure command and control within the operating shifts are
t
adequately maintained.

The program for the processing of radweste and the transportation of radioactive materials
'was examined during this inspection period. Also, your program modifications made to
.
!
Lincorporate major rule changes contained in Title 49, Code of Federal Regulations (49 CFR)f
- were reviewed. Improvements were noted in the radweste processing programs at all three
units, while your radioactive materials transportation and radweste disposal program
l
. continued to be effectively implemented.
A portion of the se'cunty program was reviewed during this period. The inspection
consisted of selective reviews of procedures and records, inspector's observations, and
,
interviews with security personnel. The inspector determined that as s' result of a
mechanical failure on two turnstiles, some authorized personnel were not properly logged-
into the prctected area. The failure did not result in a security vulnerabil;ty. The inspector
confirmed that all personnel not properly logged into the protected area were authorized to
'
. access the protected area, and that unauthorized personnel would not have been granted
access as a result of the mechanical failure.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room (PDR).
