NRC INSPECTION REPORT NO. 50-346/97011(DRP) AND NOTICE OF

VIOLATION

Dear Mr. Wood:

O.; September 29,1997, the NRC completed an inspection at your Davis-Besse site. The

enclosed report presents the results of that inspection.

Two violations of NRC requirements were identified during this inspection. The first violation

involved a failure of personnel to adequately adhere to test procedure requirements during

calibration of pressure switches in the auxiliary feedwater system. Specifically, test gauges were

used that were less accurate than required by the surveillance procedure.

The second violation involved incomplete corrective actions related to a 1995 problem

associated with measuring and test equipment specified in surveillance procedures for certain

calibration activities. The proposed corrective actions included modifying a design calculation, a

surveillance procedure, and associated instrument information sheets; however, only tht.

surveillance procedure had been revised a2 of the date of this inspection.

in addition to the identified violations, there were three instances where NRC inspectors

identified deficient conditions that had been overlookod by your staff. Two instances involved

ventilation damper position problems which had gone unnoticed by your operations staff. The

third item involved an error in ensuring the proper Technical Specification limit was entered in a

surveillance procedure.

These violations are cited in the enclosed Notice of Violation, and the circumstances surrounding

the violations are described in detailin the enclosed report. Please note that you are required to

respond to this letter and should follow the instructions specified in the enclosed Notice when

preparing your response, The NRC will use your response, in part, to determine whether further

enforcement action is necessary to ensure compliance with regulatory requirements.
