NRC INTEGRATED INSPECTION REPORT 50-395/97-13

NOTICE OF VIOLATION

Dear Mr. Taylor:

On November 29, 1997, the NRC completed an inspection at your V. C. Summer

reactor facility.

The enclosed report presents the results of that

inspection.

During the six-weeks covered by this inspection period, your conduct of

activities at the V. C. Summer facility was generally characterized by safety

conscious operations and conservative decision making.

Your return to power

following completion of the refueling outage was well controlled. We are

concerned about the two violations of NRC requirements that were identified.

The first violation dealt with an individual using chewing tobacco in a

radiation control area contrary to )osted instructions.

The second violation

involved the failure to conduct Tec1nical Specification (TS) recuired snubber

ins)ections.

Damage to TS required snubbers from transient loacing associated

wit 1 a water hammer event occurred approximately three years ago, however this

condition went unrecognized even though your TS surveillance program should

have identified the snubber failures during the previous refueling outage

(Spring 1996).

The violation is cited in the enclosed Notice of Violation (Notice), and the

circumstances surrounding the violation are described in detail in the

enclosed report.

Please note that you are requ.-ed to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response.

The NRC will use your response. in part, to

determine whether further enforcement actions is necessary to ensure

compliance with regulatory requirements.

During the ins)ection activities to review an Unresolved Item associated with

the auxiliary auilding leak detection system, questions were asked relating to

the design basis of this system.

The questions were: (1) Are the level alarm

. drains located in the auxiliary building "RHR/CS Recirc Valve Area" designed

to detect a passiv9 piping failure condition which would alarm a condition of

leakage approaching 50 gpm? and (2) Are the leak detection system com]onents

properly classified as non-safety-related since their use appeared to

associated with ensuring the capability to prevent or mitigate the

consequences of accidents that could result in potential offsite consegaences

comparable to the 10 CFR Part 100 guidelines? You are requested to respond to

these questions within 30 days of the date of this report.
