NRC INTEGRATED INSPECTION REPORT NOS. 50-369/97-15.
50-370/97-15 AND NOTICE OF VIOLATION
Dear Mr. Barron:
I-
On September 20..-1997. the Nuclear Regulatory Commission (NRC) completed an
inspection at your McGuire 1 and 2 reactor facilities. The enclosed report
presents the results of that inspection.
During _the six-week period covered by this inspection, your conduct of
activities at the McGuire facilities was generally characterized by safety-
conscious operations.-sound engineering and-maintenance practices, and careful
I
radiological work controls. Good operational control during the restarts of
Units 1 and 2 following a dual unit reactor trip was observed.
However, one
violation of NRC requirements was cited.
The violation involved failures in
the steam _ generator inspection 3rocess that resulted in steam generator tube
leakage and a subsequent unit slutdown to complete repairs.
The violation is cited in the-enclosed Notice of Violation-(Notice) and the
circumstances surrounding the issues are described in detail in the enclosed
report.
Please note that you'are required to respond to this letter and
~ should follow the instructions .specified in the enclosed Notice when preparing
your response.
The NRC will use your response in part, to determine whether
- further enforcement action is'necessary to ensure compliance with regulatory
requirements.
We are also concerned with the non-cited violation regarding your failure to
-properly evaluate the suitability of materials prior to installation in-
safety-related applications. This failure indicated poor oversight and
,
control of critical materials in containment.
In addition, weaknesses were
also noted in operator performance and post-trip review following a dual unit
- trip.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice." a co)y of
this letter its enclosures, and your response will be placed in the NRC
Public Document Room (PDR).
