The licensee will perform the following activities in non-agreement states:

Client : Control Number 000583 Connecticut Yankee (Haddam Neck) 362 Indian Hollow Rd Haddam Neck, CT 06424 Item 10:
The work will be taking place off site of Connecticut Yankee for site characterization and decommissioning (remediation) activities.

Item 12:
Any individual (s) listed on initial (and previous clarifications) form 241 application could be performing the field work. The off site work has generally been performed by rotating the personnel on a weekly basis. l Item 13:

The work location's cellular number (508) 849-7911 Item 14 A previous clarification report (November 24) listed that work would be performed from December 1 through December 31. Ilowever due to weather and client demands, work I was not performed on November 25,26,27,28,20,30, December 1,2,3,4,5,6,7 S. i 11,12,13,16,17,18,19,20,21,22,23,24,25,26,27, and 28 for a change of 30 days.

The work has no finite schedule, it all depends on the specific remediation site and the final sample results as to whether in-situ surveys will be required. There are numerous remediation sites to be completed. A clarification form 241 will be sent to adjust the days in the field.
