O Enclosed is Amendment No. 01 renewing your NRC License Number SNM-1826 in accordance with your request.

Please note that License Condition 11. authorizes you to use special nuclear materials at your present facility ::a South Miles Parkway and at your new t

facility in Edison Park. Please be advised that we cannot authorize you to O

release your present facilities for unrestricted use until we have received and reviewed a copy of the results of your close-out survey.

The survey should consist of exposure rate measurements to show that all sources of radioactive material have been removed, and contamination checks of areas where radioactive materials were used or stored. Average radiation levels associated with surface l

contamination and removable contamination should not exceed those specified in O

the enclosed decontamination guide.

Please submit the following information with your close-out survey:

A diagram of your facility on South Miles Parkway with survey and wipe test results keyed to specific locations.

O b.

The name of the person / organization performing the survey.

c.

The date the survey was perfonned, d.

The instrumentation used for exposure rate measurements and for analysis of the wipes.

The method of calibration and the date of the last calibration.

Background readings.

When submitting the results of the close-out survey, please state in your documentation, "This is Additional Information, Not A New Request".

O Please review the enclosed document carefully and be sure that you understand all conditions. You must conduct your program involvi in accordance with the conditions of your NRC license,41g radioactive materials representations made in your license application, and NRC regulations.

In particular, note that you must:

Operate in accordance with NRC regulations 10 CFR Part 19, " Notices, Instructions and Reports to WorkePsi Inspections," 10 CFR Part 20,

" Standards for Protection Against Radiation," and other applicable regulations.

Possess radioactive material only in the quantity and form indicated in your license.

Use radioactive material only for the purpose (s) indicated in your license.

Notify NRC in writing of any change in mailing address.

5.

Request and obtain appropriate amendment if you plan to change ownership of your organization, change locations of radioactive material, or make d

any other changes in your facility or program which are contrary to your license conditions or representations made in your license application and 4

any supplemental correspondence with NRC. Any amendment request should be accompanied by the appropriate fee specified in 10 CFR Part 170.

Submit a complete renewal application with proper fee or temination O

request at least 30 days before the expiration date on your license.

You will receive a reminder notice approximately 90 days before the expiration date. Possession of radioactive material after your license expires is a violation of NRC regulations.

Request termination of your license if you plan to permanently discontinue g

activities involving radioactive material prior to your expiration date.

You will be periodically inspected by NRC.

Failure to conduct your program in accordance with NRC regulations, ifcense conditions and representations in your license application will result in enforcement action against you in accordance with the General Policy and Procedures for NRC Enforcement Actions,10 CFR Part 2, Appendix C.
